I called the pharmacist at the CVS Store ID #5046 at 839 Rte 146, Clifton Park, NY 12065 on 12/5/2023 at around 8:45 p.m. I am a prescriber, and my patient could not get the medication (Clozapine) I have prescribed for him. The pharmacist would not dispense the Clozapine despite that the required lab results were already available. This is a refusal and a failure to carry out the needed medication ordered by a prescriber for this patient. This will result in treatment disruption and disease relapse.

Claimed loss: The patient is at risk for disease relapse due to the refusal and failure of the pharmacist to dispense the Clozapine medication to the patient and ensure that the patient remains stable.

Desired outcome: I will advise the patient to transfer to another pharmacy that can ensure continuity of prescribed medications.
