Taco Bell review: Food Poisoning

On 28 May 2023 I ordered Taco Bell items through the Door Dash website. My order was delivered from the 4880 West Frias Ave, Las Vegas NV at 11:31pm After consuming the food, a few hours later I was nauseous, and vomited 5-6 times. I did not consume any other food items, and or beverages after the Taco Bell food. A couple of hours later I was awoken with terrible stomach cramps. My next bowel movements were diarrhea with large amounts of bright red blood mixed in. I responded to the ER at St. Rose Dominion Hospital, San Martin Campus later that evening of the 29th. After a CT scan, I was admitted for a large intestine infection, later diagnosed as Colitis. I ended up being in the hospital and given antibiotics for 3 days and discharged on 04 May 2023. Doctors said the main cause of the infection was more than likely food poisoning. The hospital billing department contacted me and said after my Health Insurance paid their portion of the bill, I still owed them over $5800 out of my pocket. I had to continue taking antibiotics for 5 days after my release. I had a follow up visit with my Doctor on 06 June 2023. I now have to see a Gastrologist How do I file a formal complaint?
