LensCrafters review: Lenscrafters branch in Pompano FL. Refusal to fill in information on my medical claim form

I am not a resident of the USA, however for the last 20 plus years I have been using your company whenever we visit the USA to supply both frames and lenses for my entire family. My main branch was the sawgas mall in Fort Lauderdale. We have medical insurance back home in Trinidad and Tobago and as such the LensCrafters staff would have filled out the section on my medical form for the cost of the lenses and frame so that I can claim on my insurance back home. I am currently in the USA and we visited a branch of lenses crafters in pompano and the branch manager is claiming that it is against company policy to fill in the information on my medical form. Please help me here as my insurance company will not settle the claim unless it is filled out by LensCrafters.
