Supervisor was rude and refused to provide client service
your location at 3501 Cattlemen Sarasota fl has an issue with your staff.
They seem to have adopted the attitude of their day manager .
She refused to give me her full name other than YOLLANDA, NOTHING ELSE.
After being asked why I am still waiting for my appointment 10 minutes after designated time and they are admitting walk ins she proceeded to go on a rant and told me she is refusing to provide me service and told me to call cooperate.
If this is the type of employees you accept then we will do our best to inform our insurance provider to avoid QUEST labs ND SEND OUR BUSINESS TO TOHER LABS.
Desired outcome: NEED THAT SUPERVISOR SPOKEN TO IF NOT REMOVED
Overcharged, underserviced.
I have to give blood once or twice a month. My deductible has been met since April. I go to various Quest locations around Las Vegas, NV. I have no problems with any of them except the one located at 305 N. Nellis, Las Vegas, NV. They still charge a 50.00 copay. I payed in August. I went in today they tried to charge me a 50.00 copay. I told them they are wrong and Quest owes me a refund from August they told me to get out if I am not going to pay. This was a big inconvenience seeing they were late 20 minutes for my appointment and then throw me out without drawing blood.
Desired outcome: That they fix their program and return my 50.00 August copay.
SSI paperwork
I didn't have my Medicare card and brought in letter from Center for Medicare and Medicaid Services. They wouldn't accept the letter and told me to wait for billing. They couldn't copy and fax letter to their own billing to save both of us time and money.
Desired outcome: Just do the simple thing and submit letter to their billing department. This would avoid paperwork and get Quest their money sooner.
Dropping off specimen
I arrived at Quest at 10 10 am and signed in to drop of a specimen. Screen with wait times kept changing for wait times. I waited an 1 hrs and still have not been called . It is ridiculous that I have to wait this long to drop of specimen with requisition already made. This is the Quest at 110 S Macdill Ave. Suite 202. Staff was not helpful. All they could say they only had 2 people working.
Alina Estevez
Desired outcome: You should have people checking in Specimens that are dropped off.
Blood draw
The technician left a severe bruise, that's about 3 inches long from a simple blood draw. There is no way to contact anyone at the location. There is simply an endless loop of automated phone systems and no way to get a person on the phone. This person should know the pain she is inflicting and be re-trained.
Desired outcome: Contact
Driver
Driver #33790 almost ran me and my daughter off the road ! We were coming Down Franklin in Gastonia and the driver of #33790 jumped from beside us to in front of us with no signal and almost hit the front of my vehicle! I had to hurry and break and almost flipped my car! They did not have the right away and act like they did not care at all and tried to risk me and my family's life!
Desired outcome: Driver #33790 Don’t need to be driving
Quest diagnostics
Hi, my name is Mahboubeh siadat, Yesterday had an appointment and went to the closest one to our home . At the office tried to register and it said you are late. Then I showed my online appointment to one of employees and she said no it's not ours while on the page it was written Quest. However after half an hour when I said that my daughter and her children are waiting for me the accepted me. At last after a lot of delay I had to leave without my test to be down. And when I asked to whom should I complain the answer was:I don't know. What do you think?
fraud services, missing sample, no communication,
we have a 93 year old patient who needs to be catheterized to collect urine due to her catatonic state.Recently, on Nov 5th, 2021, home health nurse dropped the urine sample at San Leandro Quest. Quest San leandro has done this multiple times in the past. Quest has lost our sample about 2 times. Quest didn't result culture and Sensitivity additional 2 times. The urine report said pus and culture said no bacteria. Also, there were times, They gave fraudulent results.
I don't think they really do culture and sensitivity, they just claim they do for medicare money.
The doctor started an antibiotic on patient after giving urine sample, quest report came after 1 week and said resistant for that antibiotic but patients urine and condition got better with that antibiotic.
It is hard to collect sample. And this time Quest- San Leandro never notified us about the urine after we gave the sample, we called multiple places and then we heard from quest that it was sent it wrong container. Urine is collected by professionals.
I am a laboratory Scientist and this type of laboratory testing doesn't look right.
Desired outcome: Need to get correct results and proper service otherwise we will be reaching out to Medicare
It took way too long to be seen
I had an appointment today, and never ever had a problem. I get there and the place is full, and this never happens. I was waiting over 20 minutes with an appointment and with being 15 minutes early. I got a test to come into the office but I was already in there, so I walked into the back and not one patient was being taken care of. I asked for a...

I had an appt on 9/31/21
A previous appt where I was 20 late, I was told to reschedule and leave.
Upon returning, I observed terrible customer service from the front desk employee, Danny.
The customer service I noticed was toward another patient who arrived without proof of his referral in the computer system. Rather than actually helping the patient, Danny told him she couldn't help him instead of recommending he call his doctor's office and have them fax the referral. I suggested that - he didn't know he could do that. She had the same cold and abrupt service toward other patients, but that one stood out the most…
This location also has no surveys or business cards - as you can see for good reason.
Maybe Danny should be in a role where customer service is not required since she clearly lacks skills to help patients.
Desired outcome: Reprimand and remove
A1C Blood Test
9/28/21: A1C result reported a frightening but unlikely 12.
Either the lab screwed up... Or my CGM monitor calibration had gone incredibly bad. Spoiler: my CGM reported the accurate 90 day average of 4.61 - which was confirmed by a later blood sugar test at Quest and a finger stick at my endocrinologist.
Since 2018, I've maintained control via CGM & diet and have had normal or near normal A1C test results since then.
Quest already admitted to lab error on a different blood test at different test site the previous week that required a re-draw.
On one hand, I'd prefer to assume they screwed up this test too but and I was not willing to bet my life on it either. So Quest scared the bejesus out of me until further tests confirmed my CGM readings.
I believe either the phlebotomist or the lab mislabeled or mixed up blood vials. So some poor quest client thinks their A1C is great when in fact it is in the life-threatening range. They need to track this patient down and make good. Otherwise I consider Quest guilty of criminal negligence.
Desired outcome: Notify every one who had an A1C blood draw that day and retest them for free. Refund my insurance company for your faulty testing. Thank me for not suing you for mental anguish.
Unable to contact the billing department. Keep getting advised to call back later. I have not heard back on my e-mail correspondence.
Attempting to have the original codes submitted reviewed. Have sent revised codes twice and have paid the invoice but feel that these test should have been covered under Medicare and I should be reimbursed. Unable to manage to get resolved

Closing at 10:30 when they are scheduled to close at 12 o'clock on Saturday.
Quest Diagnostics - Sarasota University
8451 Shade Avenue
Suite 106
Sarasota, FL [protected]
The workers at this location always have a hard time drawing blood my (others also complain about this) so they rescheduled me for Saturday at 10:30. I got there stood in line with other people waiting to get in. They locked the doors with a line waiting for test. While walking to their cars they told us they close at 10:30.The also told one person who requested their referral back to go somewhere else for her test, " we closed at 10:30 and there's nothing we can do about".
Desired outcome: What ever you feel necessary. Your company is getting a bad name.
Updating Insurance
I have been trying to update my insurance with Quest since I went on Medicare as of 04/01/2021. I tried online but they only had a few private Medicare contractors; I do not belong to any of them.
I tried calling but I either am put on interminable hold or asked to call back.
I need this updated as they have a claim that Medicare should pay.
Desired outcome: Quest update my insurance and put the claim that they are trying to collect from me direct into Medicare for payment before they start trying to collect from me.
blood work that was done
date of service 02/17/20. lab code-KOP invoice no-[protected]. amount-1, 478.42 dollar.my issue is, when i walked-in there, they took copy of my insurance and my id proof . they didn't mention that my insurance did not cover quest lab. since i had my blood work done couple yrs ago i assumed it should cover under my insurance anthem. but when i got the bill it said my insurance does not cover. i was so upset about it. the whole amount my responsibility to pay. it shook me off. i am not afford to pay this myself.. i called my insurance about it. they said something blah blah... now i am receiving bill from collection department. this is not acceptable at all. if that front desk person would have told me that my insurance would not cover quest i would not taken my blood work. They are there to help the patients instead of giving trouble to them. i think, that's their responsibility.
so i am writing this with all frustration and i would need some help from the management to write off this dollars.
I COULD NOT get my report because of a failure to verify my identity and I could not get help even I have tried to contact so much.
Hi, my name is Jiahe Du and my preferred name is Heidi Du. I bought a test for the MMR antibody test which was on 1st Oct and I went to get the test on 5th Oct. The Order number is 999023 and the location is at 233 Nostrand Ave Brooklyn NY [protected]. I did not know the name of the madam who helped me to get my blood. It went smoothly until I failed to verify my identity to get my report. I could not answer any of the questions in the identity verification because they were just like asking someone else. My Quest account name is heidi du. As the first email told me, I had checked the information on my account and it appeared to be totally accurate. What's made me more anxious was that I did not get further help from the email: they did not reply to me for almost the whole week I went to the place, where I got the blood test, to ask for help, they told me to call Quest Diagnostics. But what Quest Diagnostics told me was to find my provider. They are always trying to shifting what they should do which made me so angry. Could you please help me with it? I need to get my report as soon as possible.
My email is [protected]@gmail.com which is also used to make appointment for the test. Please call me again if I did not pick it. Looking forward to hear from you!
Desired outcome: Getting my report of MMR antibody test as soon as possible.
Customer feedback for dot drug test at east point ga location
Atlanta Medical Testing Services LLC. (AMTS) is a new corporate account for Quest. We specialize in DOT drug testing for CDL truck training
I was informed by one of our CDL instructors that he went to this location on Thurs 10/6 for a urine drug screen, waited for 4 hrs before having to leave. He described this as the worst customer service experience. The staff was dismissive and disrespectful to customers.
We will not send any of our customers to this location in future even though it is the most convenient for them.
Desired outcome: Customer service training for your staff and identifying a location that will serve our customers well.
Blood draw
On Thursday, October 5, 2021 @3:15pm, I entered quest diagnostics off Grand Ave in Chino, CA. Two quest diagnostic workers (not sure if they were phlebotomists because no one there identified themselves or had any badge on that identified their title), spoke to me in an extremely rude & inappropriate manner. I had an appt. at 3:00pm that day for my...

I went to this location today for a glucose test and was treated extremely rude by the man working the front desk…He asked if I had an appointment in a nasty tone of voice and seemed agitated from the moment I came to the window…I displayed my appointment proof and he continued to speak to me in an overall aggressive tone…I asked him why he was so rude and he continued to speak to me in a dismissive manner…Another patient complained of how rude he was to her as well…If he doesn't like his job, he should find a new one instead of representing Quest Diagnostics in such a poor manner…I will ask my doctor to send me to request another location moving forward for my lab needs because this location completely turned me out off due to his unprofessional behavior…
Desired outcome: I would like this matter addressed immediately
Gave 5+ tubes of blood. 3 vials prepared for urine. Urine sample given
