Back in October 2020 I sent an email requesting a letter stating last annuity payment. I was asked via email to submit in writing which I did. I also faxed the information. In January I enquired about the letter and was told it was sent to a previous address and I should request it be sent to present address. I complied. I have made attempts weekly. On Friday, I discovered that the customer service representative was outsource to another company. In the meantime, I have lost my social security medicaid benefits because cannot get this information sent to me my case has been denied leaving me without medical coverage.

Desired outcome: I need the letter sent ASAP
