I went in to get the tooth fixed from bad work done in 2010. I told them I was embarrased to smile. They said they could fix it but that my insurance would not cover it to be redone. I paid full price to get it fixed. They had a mold sent out and it came back still looking giant. The gum part was still too small and the color of the fake gum part at the top...

Coast Dental Services reimbursments and refunds

I went to the Coast Dental location off Cascade. The service was prompt and courteous. However, there were issues in billing and I paid in full using my CareCredit Card. Insurance sent reimbursement for services to Coast Dental. I have been awaiting credit to be applied to my CareCredit account since late January. I have called the corporate location in Florida several times to speak with an operator. I am always transferred to someone's voicemail NEVER to have any call returned. It is now mid July. AVOID COAST DENTAL!

Coast Dental Services rip you off!

Where to begin... Dr. Ben Lewis is a total liar. I arrived to my appt. 10 minutes before and waited 3 hours...3 hours! After 2 hours I asked when I will get in. They said shortly the dr. is still with another patient then after another hour the dentist comes out and talks to the receptionist saying they are unable to get me in . He sees me and goes oh sorry we have been calling you all day to cancel your appt! I was livid I showed him my phone and no missed calls... He seemed annoyed and walked to the back and then the receptionist was like ohh maybe you didnt have service when we called? Umm why would you have me wait 3 hours and why wouldnt you tell me that when I arrived? Or after 2 hours when I talked to you... they put a sonicare tooth brush on my bill without asking if I wanted one... He drilled a tooth to see if it was dead? Which is def. wasn't I still have the drill hole... Then after having a crown done after my tooth cracked they forgot to floss my teeth before the cement hardened and the hygenist starting laughing going OH MY GOSH its stuck! They have to put a rubber thing in the middle and literally saw away at the middle to get them apart. After I had odors coming from the tooth ( I believe it was infected ) went back Ben Lewis said give it 6 months. No way! I went to another dentist who said their is a gap in the back of crown that is building up bacteria so I paid them to redo my crown( way better service at a ma and pa dentist). I will never go back to Coast Dental ever again! I have told many people considering them to avoid them at all cost! Now i just got a letter from the collections agency for 100 dollars and they can't explain why... I never complain online about anything but this is by far the worst dentist ever.

I believe your story as I am involved in a similar situation with Lewis. He performed, what I found out later, to be unnecessary, subpar work on me recently. He practically forced the work on me, claiming it needed to be done immediately, almost to the point of "dental bullying". After the unsuccessful, botched dental work I made several attempts to have it rectified, only to be stonewalled by lies and misinformation out of the office. Lewis himself directly lied to my face on three occasions. I am now suffering with the injury he is responsible for, and I have been informed, by several other professionals, that, at the very least, the work Lewis performed on me to needs be redone correctly by another provider, and quite possibly may lead to further extremes. Lewis has shown that he does not care about his patients. Money... seems to be his only concern.

Coast Dental Services billing and refunds

In February, 2012, I underwent a root canal at this location. At the time of the root canal, I was given a required amount to pay, which I paid on that visit. Upon completion of my insurance review, I learned that I had overpaid by about $67. I have been trying since mid-March, 2012 to get my refund. It is now mid-June 2012! Each time I call I'm told "It has been approved. Call us back in a couple of weeks for an update". Clearly, they have NO intention of EVER giving me my refund. I'm just thankful that it is not in the 1, 000's of dollars. Avoid Coast Dental if at all possible or REFUSE to pay them ANYTHING up-front.

Oh definitely, what a scam. I had to actually have my lawyer friend call them. Now, they are saying I should expect refund within 2 weeks...stated they were going through some transition. Okay, understandable but why couldn't anyone return our calls to let us know this information, if true at all!

Coast Dental Services terrible customer service

I don’t know much about the dental work actually done at this Coast Dental, nor will I ever find out. I had an appointment scheduled at the Coast Dental on 9th st in Naples, FL back in May 2012. The day of the appointment I had an unexpected car problem, and called to cancel. In June 2012 I received a $100 bill to the office. When I called to find out what it was for they said it was because of a cancelation fee, but the nice lady didn’t know much about it because she doesn’t normally work in this office and told me Dave, the “office manager” at this location would call me back. I never received a call back so I called him back. Dave was completely rude and had no customer service skills what-so-ever. I asked if we could work something out because it was an unexpected trouble and we called as soon as we knew we couldn’t make it and he had no sympathy at all and his response to everything I had to say was “No”. I totally understand that companies ask for a 24-hr cancelation, but what are you to do in an emergency situation? And $100?! We have spent thousands of dollars at one of their other locations just this year, so we thought going to one of their sister locations we would receive the same professional treatment, but this was not the case. I am so surprised that this Coast Dental is still in business with an office manager like Dave. I am sure I am not the only client that will never return to this location because of him.

Coast Dental Services billing

Hello,

We are truly very upset how smile care is handling this. We called tracy office and coast dental two months before and asked explanation why they are sending the bill for $196.Tracy office assistant told that according them we have no balance.Coast dental rep. said she will look in to it and figure out. Instead of giving explanation to us she sent it to collection agency.
This very unfair. We paid the full amount for braces for smile care, unfortunately with loss of job we moved to texas on June 2011from onwards we did not follow up with smile care for rest .
Now we paid for Apple more than $1050 from our hands to follow the braces.Smile care is holding our money.
Billing person were not able to answer my question and did the easiest job of sending to collection agency.Today we spoke to Ms. Mary Ann, she forwarded the call to collection dept.

I am going to consumer court to get the remaining amount $1050( the amount paid to Apple orthodontics Texas) from you.

Please send the check for $1050 as refund for A/c7039-10947 Patient name Jephina philip

Coast Dental Services abuse of a senior

Don't know where to start. Complained about the poor work they had done. Was abused physically and verbally.
They took my money and then just decided not to complete the work saying we will give you a full refund. Patient
abandonment. Used high pressure tactics to get as much money as they could including insurance fraud and they kicked me out. When I called back to see how much and when I would get a refund they simply refused to answer my calls.
Receptionist said if I call anymore they would charge me with harassment. I paid $4272 and never got the bridge.

Coast Dental Services disrespected intimated devalued

I entered the office to inquire if there are any cancellations for a cleaning. NO. I presented what I thought was a common complaint of frustration: "Why do Friday's have to be half days, they used to? I work 58-60 hours a week with Fridays and Saturdays off. I need the hygienst and she can only work when the doctor is present, and he leaves on Friday during mid =day. Consequently my appointment is moved several months in advance. The appointment setter empathzied and I was calm. Then this employee from behind her aggressively engaged in a verbal altercation with me stating that she works 60 hours too. I asked her why she said that, and she said, "because YOU said it." I'm thinking, why is this lady taking her persoanl frustration out on me, and then she SMIRKED. I inquired about her smirk which made me feel de-valued as a client, and she just coninued to smirk. I felt intiminated and my hands began to shake, and I needed to leave. I cancelled my future appointment, because I feel the work environment in the Venice office has a propensity to be unprofessional, and hostile. I didn't feel safe returning to recieve dental care due to that employee's inability to control her emotions. And, I have had good treatment so far from Coast Dental, to get THAT blasted at me from out of no where. Good bye, I'm not putting my care in in the hands of someone who struggles to manage their own stress and frustration in an ineffective manner.

The company provided a dental treatment plan listing work to be completed which included Elite Dentures both upper and lower and verbally stated that adjustments would be covered for six months. There was extensive bone removal and as the gums shrank the dentures did not fit. After approximately 5 weeks, I went in for adjustment and was informed that the...

Coast Dental Services dental

I have had several dentist visits and multiple services done to my teeth. In my 45years of life I have NEVER experienced more pain than I have with Coastal Dental. The night after receiving a crown I was in so much pain I thought I was dying. The pain was like contractions in my HEAD. I knew something terrible was wrong. My husband had the doctor paged. I was given a major pain medication. I was told to visit the office the next morning. Unfortunately there was an airpocket left in that tooth. Not sure what the dr. used to fill it. But...the next day it fell out. I returned to the office and was told it was fixed. It worked for 1yr. Now I'm being told by two seperate dentist that I need a NEW crown on that same tooth. Called Coastal Dental...still pending...

Stay away from this den of money-hungry vipers! It is a "bait and switch" business. This "health-care provider" is the worst I have seen in 46 yrs of living. I moved to the area, filled out their new patient paperwork and made an appointment for 8am. I arrived on time and was shown to a chair at 8:25 by the hygienist. I let the her know that I had extremely sensitive teeth and have always had to have shots just to get my teeth cleaned. She...

MARTHA COAKLEY

ATTORNEY GENERAL

October 08, 2008 - For immediate release:
Attorney General Martha Coakley Obtains Preliminary Injunction Against Individuals and Businesses Involved in Widespread Dental Finance Scheme
Thousands of Massachusetts Residents Impacted By Shoddy Dental Work, Deceptive Financing
BOSTON - Today, Attorney General Martha Coakley’s Office obtained a preliminary injunction against 15 individuals and corporations that allegedly operated high-volume dental clinics through a web of corporate fronts in order to exploit patients and obtain funds from finance companies and insurers. Under the terms of the court order, entered by Judge Bruce Henry in Suffolk Superior Court, all defendants are barred from engaging in any dental or medical business in Massachusetts. The preliminary injunction also orders the defendants to disclose to the Attorney General’s Office within five days the existence of any assets they own in whole or in part, including checking accounts, IRAs, trusts and real estate. Additionally, all defendants must disclose any other dental practices, corporations or clinics in which they have an ownership or interest. According to the complaint, filed on September 24, 2008, hundreds of patients were left with dental problems and thousands of dollars in debt after the defendants suddenly abandoned their three dental operations in Fall River and Weymouth. The Attorney General’s Office obtained a temporary restraining order, which remains in effect, that secured assets the defendants may have, barred any practice or business of dentistry in Massachusetts, and prohibited them from destroying or altering any records.
The lawsuit was filed against the following 15 defendants, eight of which are corporate entities that the defendants formed to advance or conceal their unlawful operations. The complaint alleges that the defendants engaged in civil conspiracy, fraud and violated the Massachusetts Consumer Protection Act.

Gary Anusavice, a dentist who took and maintained a leadership role in creating and operating the defendants’ scheme.
Michael Rinaldi, Joseph Robbio, Vincent O’Neill and Heather Pavao, all finance advisors and business managers at the dental operations.
Dr. Merhad Haghkerdar, a dentist that facilitated the defendants’ scheme at Spectrum Dental.
LHT Systems (d/b/a Sierra Dental), a corporate entity formed by defendants in August 2004.
Coast Dental, Inc., a Delaware corporation, which maintained a principal place of business in Fall River.
Dr. Wood, Inc. (d/b/a/ Sierra Dental), a corporate entity formed by defendants in June 2006.
Electron Marketing Inc. (d/b/a Spectrum and Coast Dental), a corporate entity formed by the defendants in September 2002; Anusavice acted as the president, treasurer and director of Electron.
William E. Salisbury Inc. (d/b/a Sierra Dental), a corporate entity formed by the defendants in December 2005.
JX Marketing Inc. (d/b/a Sierra Dental), a corporate entity formed by the defendants in March 2006.
R.P. Systems Inc. (d/b/a Sierra Dental), a corporate entity formed by the defendants in May 2004.
Arvydas Inc., a corporate entity formed by the defendants in October 2005.
Vyntautus Inc., a corporate entity formed by the defendants in March 2004.
Dr. Haghkerdar, now known as Michael Kane, no longer practices dentistry in Massachusetts, though he remains licensed by the Board of Registration of Dentistry. Dr. Kane has agreed to the terms of the injunction, which states that he must give 30 days notice to the Board of Registration of Dentistry and the Attorney General’s Office if he seeks to practice in Massachusetts. The Board has been active in taking disciplinary action against Anusavice and other dentists involved in the defendants’ operations, and has assisted in the Attorney General’s investigation.
According to the complaint, Anusavice, a dentist with a criminal history of disciplinary action, worked with Rinaldi, Robbio and O’Neill to operate as Sierra Dental in Weymouth, and Coast Dental and Spectrum Dental in Fall River. The defendants would operate the scheme by allegedly marketing “free” dental exams and low affordable monthly payment plans to prospective patients of low economic means and inadequate health insurance. The defendants set up a call-center in order to control the alleged deceptive marketing scheme and instructed their employees to lure patients into the dental clinics by “always answering a question with a question” and emphasizing the free exams.
The complaint further explains that as part of the scheme, a finance advisor was introduced into the patient-dentist relationship with the intention of “closing” a patient with credit obligations so that the dentist could start immediate, often unnecessary and costly treatment, and preventing the patient from considering alternatives. “Closing” a patient meant that the advisor had successfully completed the new patient protocol, which involved putting the patients into an operatory chair, bibbing them, explaining the treatment, introducing the dentist and running a credit check or pre-qualifying the patients before writing the treatment plan. The advisors, who were paid on commission, often proposed more extensive treatment than was necessary to generate larger fees, including unnecessary bridge and root canal work.
According to the complaint, rather than the low monthly payments promised to the patients, the loans arranged were essentially credit card loans that carried monthly interest rates as high as 22.98%. In hundreds of cases, the defendants collected thousands of dollars per patient from finance companies without proper disclosure or agreement from the patients, leaving the patients with substantial debt for unnecessary and often unauthorized dental work. For patients with dental insurance, the defendants allegedly submitted false and inflated claims to the insurance companies and billed the patients in full. Although the treatment was generally paid for up-front, many patients were left with incomplete and/or shoddy work when the defendants abandoned their three dental operations without notice. The defendants used the corporations they set up to continue their scheme even after the lenders cut off access to the defendants.
One of the defendants’ alleged operations, Sierra Dental in Weymouth, abruptly ceased operation in December 2007, without any notice to patients, leaving abandoned patient records, as well as prostheses, x-rays and other material patients had paid for but had not received. This followed similar closures of Coast Dental in Fall River, which took over operations of Spectrum Dental at the same location following an investigation of the Spectrum Dental operations by the Fall River Police Department. Under Massachusetts Law, dentists are required to preserve the confidentiality of patient records and to maintain records for at least three years from the date of the last patient encounter.
The Attorney General’s Office received over 200 complaints from the victims of the defendants. Both the Attorney General’s Office and the Board of Registry in Dentistry have previously taken action against Anusavice and William Salisbury, a dentist who worked at Sierra Dental, and both have lost their license to practice dentistry in Massachusetts.
The current action by the Attorney General’s Office seeks penalties, restitution for injured patients and a court order permanently prohibiting the defendants from maintaining, operating or owning an interest in any dental practice. The investigation is ongoing.
This matter is being handled by Assistant Attorney General Jeffrey Clements, Chief of Attorney General Coakley’s Public Protection & Advocacy Bureau, Assistant Attorneys General Gillian Feiner, Jeanne Veenstra and Thomas O’Brien, and Mediator/Legal Analyst Rebecca Peyton of Attorney General Coakley’s Consumer Protection and Health Care Divisions Quinton Dale, Chief of Attorney General Coakley’s Investigations Division and Investigators Monique Cascarano and Dante Annicelli are assisting in the case.

I called Coast Dental to have my teeth cleaned. They informed me they would have to do an xray before they did the actuall cleaning. I asked them several times on the phone if after this procedure they would do the actuall cleanin they assured me they would. When i got to the dental office i again asked them if after the process would they clean my teeth, iasked them several times and they again said they would. After the xrays they then informed me i needed to do several hundred dollars more worth the work before they would proceed. They said they could not do the cleaning because of other conditions it was against Florida law. Ihave emailed the office and sent registerd mail to them to explain to me what law it was against. I have never recieved an answer to this question. I am taking them to court to sue them for what i believe is fraud. They have filed a Motion to Dismiss against me. This will take place on Feb. 17th 2011 at 8:45 am im District Court at 930 East Parker St Auburndale Fl. Plese feel free to attend

The front desk staff can be very informative and helpful! But the dr's and nurses try to push services and overcharge! The dr only heard half of what we said and assumed, I guess because he thought we were well off! As with alot of ppl in the eonomy right now we can only afford basic neccessities to live atm!

We had to borrow money to get his tooth pulled and the date they gave us to actually get it pulled was almost a mo away!

We go to the appt and they say irrigation is mandatory and not an option! an extra $28 for the irrigation, plus an extra osha fee! then it was to be a surgical extraction which they charged us for and it was a simple extraction! We now have to find a way for gas for him to get back and forth to work this wk now because of it!

a dental office is supposed to be just that. not a boutique or sales office!

I read the reviews after I made the appt for my husband and almost cancelled I should have. My husband is 62 years old and had not been to a dentist in 20 years. they said his teeth were not bad. then they proceeded to finish the exam and hand him an estimated bill that amounted to over $6, 000.00 dollars that our insurance did not pay. The statement also said that was an estimate and could end up more. All we want is for them to take care of the teeth that were bothering him and maybe a cleaning. I guess he does have some periodontal disease so they want to laser his teeth and give him all kinds of treatments and come back every 3 months so they can do whatever. I guess you know we are not going back, I will be lucky to get him to go to another dentist. Thanks Coast Dental you really didn't help. Thank You to the people who wrote reviews for because of them I told my husband not to sign any papers until I saw them. I didn't want him to commit to anything but I shouldn't have worried because when he saw the bill he came out and said there is NO WAY! I would not recommend them to anyone. Oh this was the one on Lakeland Highlands Rd, Lakeland Fl

I had extensive dental work done in 2004 at spectrum dental in Fall River. All my teeth were ground down and porcelain caps were placed over them. The appearance of my teeth is great .I've had no problems eating. The only problem has been when brushing .My gums will bleed almost every time. Slightly. Because my teeth have been problem free I never went back to Spectrum . I did notice they had changed their name to Coastal Dental. Recently I've developed a tooth ache but now knowing about the Preliminary Injunction filed against these people I can't get it taken care of.
Is there any way I can get my records back so a legitimate dentist will know what he's dealing with under all those caps ?

I just experienced the same situation as this auburndale resident and wish to find out where florida law says a dentist can't give you a regular cleaning because they think you need a deep cleaning. my e-mail is slb45@earthlink.net if anyone can help. My former dentists had instrumeted/electronic machines to give me my periodontal chart and I received copies of them and still have a copy of one from 4/04/2000, so electronic equipment existed atleast 11 years ago. Why would you want to trust someone poking a pick in your mouth when all modern dentists use technology. TheirX ray equipment certainly was way out of date and this dentist I had was taking a picture of my tooth with his camera with a zoom lens on it and than showing me the picture on the camera as he held it in his hands . I accidentally got to see a few of his freinds as he flipped too many pages, PLEASE. This dentist group needs to be ...

I went into Coast Dental for about my 10th time today, 12/17/11, for an exam and xrays. The lady takes my xrays and i wait the usual forever for the hygenist to come exam my teeth. My first problem:the hygenist comes in, tells me all the number details, and begins jabbing at my teeth with no regard to how much/hard she's stabing me.My second problem:she spouts off nothing but 2, 3, and 4's.Only one 1.My third problem:after i sit there, gums bleeding and head swimming, for however long, the head dentist comes in and checks my teeth. She looks around and tells another lady all these things i dont understand and procedes to tell me what the hell she just said when she got her hands out of my mouth.Apparently there's three crowns i have that need replaced because "there's a little decay."Mind you, this is my 10th time there, in which the 9th wasn't but 3-4 months ago. They would've clearly seen said problems with these crowns before. These crowns they talk about?I've had them since i was 8, and im 17 now. Finally, my fourth problem:they tell me i have SIX cavities.BULLCRAP.Everytime i go there, they've told me i have at least 3 cavities, if not more.Might i include that my idetical twin sister went as well for a cavity filling and a "super cleaning" and they began by numbing the worng side of her mouth for her cavity, then numbing her up to her eyeball on the correct side, and absolutely choaking her with baking soda treatment for her cleaning? You can bet we'll NEVER go there again. And my advice?Consider ripping out all your teeth BEFORE you consider going to Coast Dental to have them practically do it for you.

I would like to share about my dental experience with Harlingen Family dentistry services. I wanted to have a better smile and I got 4 veneers in front, my wide space was closed in and I was so happy to have them done. Each visit had me in the chair for at least 4 hours! I felt that after all this time and money that I paid, I would not have to go back. Well needless to say, the veneer broke and I had to go back again to have a temporary and several uncomfortable impressions made. The doc told me the lab guy was going to use a better quality resin and I proceeded to go through the grueling experience of chiseling off the temporary and another 4 hours of trying to get the tooth in place. I started these procedures in 2009 and all veneers had to be replaced! Not only the time spent in the dental chair and embarrassing experiences of chomping down on you broken tooth, my teeth are not properly aligned and I have gaps that get food trapped in them. Oh and the last one that broke, the dental assistant didn't know how to make the temporary and it fell out the next day. The doc is tired of seeing me and I told him that maybe I was not a good candidate to have veneers. He then told me that I would need caps or crowns! I told the assistant and doc that I was not happy with the botch job and asked who I can talk to to file a complaint.
I voiced my dissatisfaction and the doc is the head secretary who handles the business. I told her that I wasn't happy with the results and I wanted her to ask him to return my money so that I could find a competent dentist. I told her that I would like some feedback and she just fluffed me off, didn't even write my complaint and told me she would talk to him and respond back to me. I have not heard a word and I don't feel that I was treated with respect and got the oh well impression. Do I as a patient have any rights? What can I do ?

Coast Dental Services unethical

The hygenist is deceitful. She explains procedures she is going to do and has the patient sign that the work can be done. What she fails to explain is that most of the procedures are NOT covered by insurance. This office is all about making money not providing services to the patient. The dentists are just as bad. The dentist said that I "had" to have a filling replaced in a tooth. Then he said that the type of filling he "had" to use was not covered by my insurance. I was charged $300. I later took my daughter to another dentist. She had the same tooth filled with the same material using the same insurance. The cost for her filling was only $17! Coast Dental will not use procedure codes that insurance will accept. I am telling everyone to avoid Coast Dental!

Dr. Dale Salomon (if you want to call him a Dr) is the worst dentist ever. He is rude and lazy. I was scheduled to be seen at 10:40 am and wasnt taken to the back till 12:50 pm and then waited in the chair for an additional 30 mins while watching all the employees eat lunch. When I was finally seen Dale was very rough and rude with my mouth. He asked...

Coast Dental Services getting something they did not explain then make you pay for it, and overpayment

I went in to have a tooth pulled that was killing me with pain, they did an x-ray on that tooth and pulled it. Then when i checked out I paid $431.80 after insurance. I went back for an appointment to have x-rays and cleaning and an evaluation (8, 000.00 without insurance), they never cleaned them and did 3 different kind of x-rays that insurance said that is not allowed all in one day. They had me sign a paper about x-rays not telling me I had to pay. They told me insurance will pay x-rays and cleaning 100% ok. Checked out charged me $20 for the set of x-rays, then charged me $28.30 more because the insurance EOB was short that money. I called the insurance and it was a lie, the EOB showed I paid more the first time to begin with. Now they want to do a root canal on a tooth that is not even there. Then they want to put a white porcelain crown top front and his teeth are a yellowish tint and permanent from a baby cant change that without capping every tooth NOT, How ridiculous is that. WARNING they are out for your money and nothing else. the bottom line they owe me money and I want the overpayment back. LOSERS

Coast Dental Services unessasary service

I too was told I needed 2 different types of xrays-a first in my life. I agreed, had a consult with the dentist and the hygienist who never said anything about my having gum desease. Came back 2 weeks later for a cleaning which was done by a different hygenist also never mentioned gum desease. Go back 6 months later for routine cleaning and am told by a third (new) hygenist that I might need a scaling for gum desease. She proceeded to forcfully probe my gums causing bleeding, which I don't normally get and says I need $500 worht of periodontal work. I asked why the previous visit with the dentist and 2 other hygenists never mentioned this and that I couldn't afford that and to just let me get a cleaning--she stated it was illegal. I had been seeing a (different) dentist every year and getting cleaning every 6 months and was never told this so I left. A month later the "office manager" calls to say the dentist was concerned that I didn't get the procedure done and when I explained why she got in to a nasty argument with me saying the previous people didn't do there job right (same dentist)and the dentist can see it on the xrays. I told her they took 2 sets and the dentist nor the other 2 hygenists mentioned gum desease. After contradicting herself several time and debating with me for a half hour basically hung up on me.This place is a big rip-off and I won't be going back.

These people are totally unethical - do not ever go there! They try to tell you that you have major dental problems and want to charge you way above and beyond what your insurance covers. Stupidly, I went there twice thinking the same thing would not happen the second time-wrong. They tried to tell me I had major dental problems and they could only do...
