Subject: Formal Complaint Regarding Medical Malpractice

I am writing to formally file a complaint regarding what I believe to be two incidents of medical malpractice that occurred at your facility. I ask that you please read and respond with an action plan.
This matter started in August 2021, at which time I had prostate cancer surgery that was performed by Dr Keto. You will find by reading my medical history that this surgery has since resulted in many ongoing medical issues. I won’t go into the long list of details – but ask that you read my medical reports and you will clearly see what a trainwreck my life has been since this surgery occurred back in 2021.
Fast forward to March 2025, at which time I was seen in the office by Dr Piotrowski and it was determined that a catheter needed to be inserted. About an hour after I left the office, I was in intense pain and had a lot of bleeding. I immediately went to the ER at Aurora in Manitowoc where they notified me that the catheter had been accidentally inserted by the doctor into my rectum. I was then transferred to Baycare in Green Bay for further care. I was told on several occasions (by your very own staff) during this hospital stay that I have a very strong case for medical malpractice and should definitely consider pursuing legal action. Each doctor and nurse showed great empathy toward my unfortunate situation that was caused by your facility.
I would like to say that Dr Piotrowski has been nothing but professional during this whole matter. He immediately admitted fault and showed empathy toward the situation (especially knowing what I have already been through over the past several years). In fact, my only hesitation in filing legal action is because of his compassion. He has a heart of gold and I appreciate that greatly.
Having said that, I also need to think about myself during all of this. I feel like I have been through hell with all of these issues that your facility has caused over the past four years. You have not only caused me great financial stress, but also mental stress. I have incurred a lot of unnecessary medical bills and have been forced to miss a lot of work – both causing a huge strain on me financially over the past four years. If that’s not enough, my emotional state has deteriorated greatly and my family has become very worried about me.
Also as a result of this negligence I experienced while under your care, I will be suffering ongoing and future medical complications, including possible bladder removal and reconstructive surgery. I have a very strong possibility of living with either a urostomy pouch or a colostomy bag. These treatments will incur more medical costs, time off work, and cause more emotional and physical distress.
I have obtained both independent medical evaluations and legal counsel, and I have been advised that the circumstances surrounding my case warrant a malpractice claim. Again, I am hesitant to file a suit because of my loyalty to Dr Piotrowski– but also feel it’s time I need to take action. I am prepared to follow through with hiring one of these attorneys but am hopeful we can come to some sort of agreement before pursuing action.
I am requesting a thorough internal review of this matter and a written response within 14 days explaining what steps the hospital intends to take to address the issue. I am hopeful you will do the right thing and offer me some compensation to offset the lost wages, medical bills and emotional distress that this has all caused me.
