Reference trips number 490-[protected]-00 and 490-[protected]-00. I did not authorize either of these trips nor did I sign any consent or document requesting these services. Therefore I cannot be held legally liable for these claims. The first of the above was ordered by the Federal Way, WA police department on January 13, 2020. I have no idea who arranged the second trip from Saint Francis Hospital in Federal Way to ManorCare North in Tacoma, WA. The ambulance team simply came to my room in the hospital and wheeled me out on a gurney to the vehicle without a word, and drove me south to Tacoma. I was taken in on the gurney to my bed and then they departed without comment or request for authorization or signature. If Saint Francis Hospital in Federal Way authorized that trip then they, not me, are liable. The Federal Way police report number is 20-487 from Officer Brad Chaco. on January 13, 2020 in the evening. You have the date of January 20, 2020 for the second trip from Saint Francis. That is all I know. Some of my numbers of interest to you are (1) Washington Quest EBT card issued by WA State Agency Dept. of Social and Health Services {DSHS} [protected], (2) State of Washington Services Card (Provider One) 102123164WA, and (3) I do not wish to give you my Medicare number here but you should have along with my Premera Blue Cross numbers. I do believe that I have Medicaid and that should cover any services billed to you. I am of very limited means with no assets and any recovery effort by you against me would be fruitless and indeed without basis as I am not legally liable. You should be able to recover from the Federal Way Police Department, an agent of the City of Federal Way, and secondly from Saint Francis Hospital or whomever they have on record as having authorized that trip.
