You did initiate a payment to our billing office on 12/19/2019 for the balance owed of $151.71 for date of service 11/12/2019. Unfortunately, this check was never received by our office and, therefore, was not applied to the account. Per our record we did continue to bill you, multiple statements, for this amount. Those statements were mailed on 1/3/2020, 2/3/2020 and a final notice was mailed on 3/5/2020. We also contacted you by phone on 2/19/2020 at the phone number that was provided to our facility and confirmed by you on the date of service. There was no answer, so we left a voice message requesting a return call regarding the outstanding balance. I searched our call history but was not able to locate any attempts from you trying to reach our office to resolve this matter. Upon receiving a copy of the cancelled ***/***, check No. XXXXXXXXX, on 10/26/2020 our payment posting department searched our entire system, in an attempt, to locate this missing payment. We were not successful, therefore, we contacted ***. The representative we spoke with confirmed the check was never cashed and the representative informed us that the policy holder (***) was notified of this prior to the file being placed with collections. That same representative also stated that it would have been the responsibility of the policy holder to request a new check be issued. Per ***'s records, there was never a request to reissue a check. Our payment posting supervisor contacted Mr. *** on 10/27/2020 and discussed this matter with him. I contacted Mrs. *** on 10/28/2020 but had to leave a voice message requesting a return call - No return calls have been received from Mr. and/or Mrs. as of 10/30/2020. Due to the overall miscommunication on both sides and because you did attempt to send us the payment promptly after getting your first billing invoice, we have closed the account with collections and it is on hold with our office. We will agree to hold the account in office until 11/30/2020 before further steps to collect the debt are enforced.
