On 11/9/2024 in San Pablo California I had oral surgery.
The oral surgeon seemed to disregard my allergies stated on my form.
This oral surgeons ignoring of my allergies could have caused me great bodily harm.

On my first visit I let the receptionist know through filling out my dental history.

On 11/9/2024 my day of surgery the oral surgeon injected me with novacaine without first consulting me about my novacaine allergy
