Quest Diagnostics review: Denied service

I want to file a complaint, I was denied service because I cant wear a mask, I asked for paper work where it states a patient has to wear a mask to receive and they could not provide, they are rude, stressed, more worried about people wearing masks than doing their jobs, there is no law for masks, they are not mandatory to receive treatment, there are no studies that require somebody getting blood work needs to wear a mask, covid virus is not even isolated to force mask wearing, these people infringe on basic human rights and have no paper work to force their will upon patients, how can I file a formal complaint?

Desired outcome: I would like to sue this company for denying me blood work and treating me like a second class citizen, I am a disabled senior and nobody should be treated this way, I doubt this will happen but it would be nice to get my blood work
