I've been experiencing ongoing issues with University Dental Care regarding a billing error on my account

I've been experiencing ongoing issues with University Dental Care regarding a billing error on my account. They have been consistently reporting a charge of $201 that does not belong to me since December 9. This situation is a clear violation of the "HIPAA" Medical Privacy Act, as well as the FDCPA, FCBA, and FCRA. To make matters worse, the same account has been reported twice on my file, under the name "***". I have no knowledge of the account number, and my social security number is ***. My date of birth is ***.
