Dear China Eastern Airlines Baggage Service Team,

I would like to report a checked baggage item from my recent flight with China Eastern Airlines.
When I arrived home after my trip, I discovered that my suitcase was damaged.
Please find my flight and baggage details below:

Passenger Name: KIM, HWAYOUNG
Flight Number: MU5041
Travel Date: 07 March
Route: Pudong (PVG) → Incheon (ICN)
Seat Number: 37L
Baggage Tag Number: MU3781196953
Number of Bags: 1 (as shown on the attached baggage tag)

Description of Damage:
The edge of hard-shell casing is cracked
The bag size is 28inch carrier
I kindly request the Damaged Baggage Report form and instructions on how to proceed with the claim.
Attached are photos of the baggage tag, boarding pass, and the damaged suitcase.
