I hope this letter finds you well. I am writing to address a matter of concern regarding an incident that occurred during my recent visit to your car wash facility, Chevron, located at 1704 Saratoga Ave, San Jose, CA 95129. On 09/11/2023 around 7:30 p.m., I brought my vehicle, a white Audi Q5, for gas and a carwash.

Regrettably, during the car wash, an unfortunate incident transpired that resulted in damage to my vehicle. Specifically, the back passenger-side panel was removed in the cleaning process, and it sustained various forms of damage, including a scratch, and a noticeable dent.

I immediately reported the incident to your staff, who let me know I could record the footage and take photos of visible proof.

Considering the extent of the damage and the potential costs associated with its repair or replacement, I kindly request that you initiate an insurance claim to cover the expenses incurred in rectifying the damage to my vehicle. I understand that accidents can happen, and it is for this reason that businesses typically carry insurance coverage to address such situations.

To facilitate this process, I would appreciate it if you could provide me with the necessary information to initiate the insurance claim. This may include your insurance company's name, policy number, and any contact details for your insurance representative.

I kindly request that you respond to this letter with the requested insurance information and any other relevant details regarding the claim process. Your prompt attention to this matter is greatly appreciated.
