I am writing to formally raise a concern regarding a crown replacement procedure performed at your Tukwila office in 2024.
The crown began to chip from last couple of months and has since sustained significant additional damage while I was eating on May 27, 2026. When I contacted your office, I was informed that a replacement would cost approximately $1,200, as my insurance does not cover a repeat procedure until 2030.
I find this situation concerning, as a crown is expected to last significantly longer under normal use. The premature failure raises questions about the quality of the material and/or workmanship. Given these circumstances, I request that:
The case be reviewed for potential material or procedural defects
The crown be replaced at no additional cost to me
In addition, I would like to highlight a gap in communication. I was provided with a contact number (ending with XXX-XXX-9860) to reach Feven; however, my calls have consistently gone to voicemail, and I have not received any response despite leaving multiple messages.
I would appreciate your prompt attention to this matter and request that someone from your team contact me to discuss the resolution steps at the earliest convenience.
