Arizona Medical Board Lodge at sedona

It distresses me to even have to write this letter and it is a sad commentary on the Sedona Lodge that they have refused to work with me to resolve this issue. On Thursday October 1, 2020 I contacted the Lodge at Sedona about attending their water fasting retreat center. At the time I contacted them, I informed them that I have serious medical and mental health issues - Ehlers Danlos Syndrome and active bulimia and anorexia. I was worried about the impact fasting might have on me and my ability to do it given my medical conditions. The person I spoke with informed me that their treatment would be highly effective for my medical conditions. I have since learned that offering medical advice to ill people sight-unseen over the phone is a breach of medical and professional ethics. Anyway, I expressed my hesitation to go to the Lodge but also my desire to do anything to help my health and at that point the person I spoke with advised that if I wanted to secure a low-price room rate I should give my deposit over the phone. I asked what would happen if my doctors and/or treatment professionals told me fasting was bad for me given the eating disorder and they told me that they would of course refund my deposit. He told me this several times and I actually called several times over the course of a week to confirm prior to putting down a deposit. He processed my card payment over the phone without me ever obtaining paperwork or a contract until after processing my card payment and what he told me prior to processing my credit card payment - that I could cancel at any time - did not correspond with the documents he sent AFTER charging my card $2, 379.93. He stressed that it would be easy to cancel and get a refund but that, if I wanted to obtain a discounted rate, I needed to "pay now." Obviously, this was nothing more than a sales tactic and very manipulative but I had no way of knowing this since I did not receive documentation until after he took my money. I am including my telephone bill from October where you can see an 18 minute phone call at 9:19 am on October 1st during which he took my card payment over the phone. I received the email payment confirmation at 9:45 am and this was the FIRST time they stated that deposits were non-refundable and this was in direct conflict with their prior verbal promises. You will also see evidence of a phone call on 9/29/2020 and numerous phone calls the week before they processed the payment because I wanted to ensure that, prior to paying them, they knew my medical status. I had informed them that I did not have a doctors' appointment for several weeks, but they advised that it would be better to put down a deposit and secure the current rate and that I could still consult my doctor to see if it would be counter-indicated to water fast and let them know afterwards if I was able to come. They stated that I would absolutely be able to cancel if my doctor informed me that fasting was not advisable. It was after they said that and they processed my payment that I got the paperwork they sent stating that all deposits are non-refundable. That paperwork was sent at 9:45 am and is included in the documents provided. I contacted the Lodge again because their email contradicted what they had told me MANY times on the phone and again I was informed that they would allow me to cancel if my doctors objected to water fasting based on my medical conditions. After that, I spoke to my therapist about my desire to attend and she HIGHLY recommended that I cancel. She advised that attempting to fast could kill someone with my medical issues and I then contacted the Lodge and told them I couldn't come as planned. They canceled my reservation at that time and did not put in a future date for a future reservation. I notified them well within the cancellation range parameters that I would not be coming and I spoke to two people at that time, both the front desk person and a doctor on staff. They cancelled the reservation but advised me to wait to cancel completely and process the refund until after I spoke to my medical doctors. In the meantime, they converted my reservation to a gift card so that the money would remain on my account. I want to point out that I contacted them in mid-October, a MONTH ahead of my initial November 16 reservation, because I wanted to ensure that I was well within the 30 day cancellation window. They told me it was no problem to cancel my reservation, which they did over the phone, and they said that I should seek a doctor's opinion prior to deciding to get a refund altogether. They assured me, again, that they would defer to my doctor's recommendations and even made a special point of reminding me about the lab work they require for people to embark on a water fast. I did what they suggested and a few weeks later I went to see a doctor, taking Sedona's blood testing forms with me, and the doctor told me that fasting could kill me or severely exacerbate my health issues and there was no way I should go. He advised me to cancel immediately. After that, I followed up with yet another doctor who told me the same thing and told me it would be medically irresponsible and negligent for anyone to suggest someone with my history would be helped by fasting. I even contacted True North, another fasting center, and the doctor there told me that given my history he thought fasting would be severely damaging to me and that under no circumstances should I do it, now or in the future. Following all of this data-gathering, I contacted the Lodge at Sedona and spoke to two very kind front desk people both of whom suggested that, given my history, a refund would not be a problem and also considering the promises that were previously made to me, they would be happy to help. They then transferred me to the medical director whose behavior was deplorable! He told me he would not give a refund despite the fact that fasting could kill me. I reminded him that I gave my deposit over the phone and was not made aware of the cancellation policy until after the card payment had been processed and I begged him to issue a refund so I can utilize the improperly obtained funds to seek proper care/treatment for the ED. I told him what my doctors and therapist and the director of True North had told me about the dangerous consequences to fasting. I pleaded with the doctor and told him that an attempt to fast could outright kill me and he basically said "So what, that's your problem?" He was cruel to the point of being heartless. I did NOT at any time receive ANY services. I was lied to and misled and they are attempting to steal money from me. Nothing about their behavior is conscionable. I am including along with this letter proof from my phone bill that the deposit was placed prior to them emailing me anything saying they had a no-refunds policy and 2 doctors' letters as well as several online articles demonstrating the dangers of fasting to individuals with eating disorders. I am also including information about Ehler's Danlos Syndrome, the connective tissue disorder that causes me severe GI distress and joint problems. My doctors also advise that fasting would be bad for EDS and could trigger terrible symptoms and lifelong ramifications. The Lodge at Sedona should have known this or, at the very least, should have stood by their promise to honor the medical and mental health advice of my doctors! They made false promises, dispensed incorrect medical advice, and have not provided any services to me at all. I really hope this organization goes out of business as their behavior is putting people's lives at risk.

Desired outcome: I would like them to do as promised and refund the $2379.93 to me

Arizona Medical Board Dr. Kristy Rae ingebo gross misconduct

This doctors behavior is unethical and very disturbing to me that she is allowed to practice medicine around anyone she not only lied on medical documentation she also lied to Detective Britt to obtain criminal charges with on me, she exasperated my sons newly diagnosed condition (By Dr. Hussam mullah) by enforcing a no food and drink order. Drugged up my son so when officer Cobbley came into the hospital to take pictures that is what the officer saw. It is evident my son was held down and drugged before the officer came to take pictures or my son would have been screaming to go home. Her actions have ruined my sons life and mine. My son she said ruined my life as if she could speak for me. I have audio of her pressing for my prosecution and for jail time so she could tell my son Jonathon he was never coming home I have the records pictures everything to prove what I am saying is a fact. She should be permanently stripped of her medical license and never be able to do this to another innocent family ever again! Attached is the exasperated condition that this doctor and medical staff inflicted on my son and the pain they put my son through and myself I would send audio files but would have to email them to you my email is [protected]@gmail.com phone number [protected] this woman abused my son and she should be held accountable for it. Arizona Medical Board Medical records for Richard Martinez 8512 E. Calle Bolivar Tucson AZ [protected]

I, Richard Martinez, was a patient at Desert Pain & Rehab in Tucson Arizona for nine years with no problems then I began to be seen by Donald Seidel, PAC. He began to tell me to hoard my medication and take my meds other than the way they had always been prescribed. I complained about him and said that I no longer wanted to be seen by Mr. Seidel. After my complaint I was sent a termination letter which sighted that I WAS NOT taking my meds as prescribed. I have fractures in my back and tailbone along with other maladies that are very painful. I signed a release for my medical records to be sent to my primary care doctor so I could get a referral to a new pain mgmt clinic, I also signed a records release there. That was 6 weeks ago and Desert Pain & Rehab has yet to send my records dispute numerous requests from my doctor or the pain mgmt clinic I was referred to. I cannot be seen or obtain the medical services I need without these vital records and Desert Pain & Rehab is refusing to hand them over to me personally sighting office protocols. PLEASE help me as I am quickly running out of my medications and will soon need to go to emergency risking my already compromised health during a world pandemic. You can also consider this my formal complaint against Desert Pain & Rehab and against Mr. Seidel PAC. I have also filed a grievance with Banner health insurance (they have also requested my medical records on my behalf to no avail.) Thank you.

Arizona Medical Board Transfer of medical records from yuma md to tucson md

Today's Date: December 10, 2020
I was advised by an AZ Medical Board Intake Officer to indicate that this is my second complaint if I Dr. Shehata has not transferred my files to my new doctor (IORA Primary Care - Tucson) by early December.

First complaint 11/13/2020 indicated the following information:

Greetings, I moved from Yuma, AZ December 2019 to Tucson, AZ.

March 2020, I established patient care in Tucson, AZ with IORA Primary Care, 8616 East Broadway Boulevard, Tucson, AZ 85710. [protected].

March 2020, I signed forms at IORA authorizing the transfer of my medical records from Dr. Hazem Shehata, 1881 W. 24th Street, Suite AZ, Yuma, AZ 85364. [protected].

According to the documentation of IORA PRIMARY CARE, requests for my medical records were sent to Dr. Shehata's office beginning March 2020 and several other times after that until October 16, 2020. As of today (12/10/2020), more requests have been sent to Dr. Shehata's office. I and the medical staff at IORA have talked with Morgan and other staff at Dr. Shehata's office who either promise to send the records, have requested another signed authorization - which was sent, claimed their fax machine is broken, etc.

To this date (11/13/2020) IORA Primary Care has not received my medical records from Dr. Hazem Shehata, Yuma, AZ despite various requests.

Without these records in their possession it will be impossible for me to obtain proper medical treatment from my new primary care doctor at IORA.

Arizona Medical Board Allergan natrelle style 68hp saline smooth breast implants

2019 REPORTED LEFT IMPLANT CAPSULAR CONTRACTURE

Allergan told me implants don't get you sick, and said I did not have to get removed as women walk around for ten years after these issues with no problems. I was told this same story both times by a gentleman. I finally realized this is far from true.

I became chronically ill/disabled over following decade, both ruined, rupture, capsular contracture, right one slipped into skull, tumors grew into body, spine, cerebrum, neck, shoulders, all over, silicone poisoning from shell, severe cognitive dysfunction led to temporary psychosis last year leading to 6 day hospitalization, nearly killed me, my body is draining all torturous tumors, ended up using wheelchair, cane, walker, unable to work and use college degree, had to put my ministry on a sabbatical, life destroyed, allergan kept telling me implants don't make you sick. This is not true. The smooth are just as dangerous as the textured. So many lives destroyed. The silicone in the shell leaks out causing poisoning. The biofilm reacts to the implant regardless if smooth or textured. Please do something and save lives. Thank you. Oh, the draining feels like brillo and chards of glass being pushed through the veins including eyes. Sincerely, pastor lisa. With god all things are possible - matthew 19:26. Stand for truth. He did not listen. I went to him right away, three times after surgery. The first two times I was given pain meds for excruciating pain and the third time, he refused to see me, which began my spiral into the abyss of hell.

I made an appointment for my annual free wellness visit for 3/25/20 with doctor holubowitch and he charged me for the visit and now tells me that they cannot resubmit the bill. Arizona Medical Board Infectious disease

Dr Ampel in 2109, did not evaluate me properly based on information given to him. Specifically, I had an active Lyme and Relapsing Fever Borellia which went undetected until results in Dec 2109. By Martha Grout in Scottsdale.

The only 2 doctors who would help me were homeopathic m.d.s in AZ and NV.

I told him I had been bitten by ticks and other wild animals and had horribly painful obvious visible symptoms.

I tried multiple times contacting him thru portal even to ask for diagnostic and pain advice help cause it got worse.

I tried to make appointment with chief infectious disease doctor at Mayo Phoenix location. He refused would NOT SEE ME for 2nd opinion.

Dr. Ampler sent me home with stage 2 Lyme and Relapsing Fever.

But I figured it in November..

Two years before my visit, I put myself on antibiotics thru another doctor. I had gotten better since then but HE WOULD NOT BELIEVE ME !

Arizona Medical Board dr pete coury on southern ave in mesa, az

Dr. Pete Coury's office deleted my health records and now have no records of what is wrong with me when going to Dr.'s. I don't need to go get all tests again for lack of compency. They said they were only to have 20 issues on my portal so they erased the rest. They even blocked me from getting back on my portal again on line. Can anything be done? As, I stated all my back issues and other items are gone. He has back pain but no identity like he had before from my Mri and catscans and xrays.

Arizona Medical Board extreme time to review a complaint

My complaint is with the Arizona Medical Board. I filed a complaint against a Tucson doctor in February of 2019. I have called every two weeks to check on my complaint. I receive the same answer every time.
They are still gathering information. I find it hard to believe that it takes 6 months to gather information.
This State Agency answers to nobody. I contacted the Governor's office and was told even the Governor has no authority over them. Is anyone else having this problem?

What is wrong with our judicial system? I have engaged in some very interesting conversations with several well-known trail attorneys. Believe it or not they all said that even though you prove beyond a shadow of doubt that a physician has committed a blatant error or committed an act deserving disciplinary action against him or her, they have the right to refuse or admit wrong doing thus negating and all responsibility to compensate an individual who was harmed by their actions. They must sign off on any allegation and say they are accountable for the wrong doing. Is this justice ? The Arizona Medical Board condones with policy. Why do physicians have malpractice insurance? They are insured but you cannot collect any settlement due to the fact they will not admit any wrong doing. The worst part is that many of these individuals are literally killing their patients. I guess they have a license to kill, without any chance of being prosecuted. And yes the Arizona medical Board knows all of this. Totally corrupt and definitely not a State agency that protects the public. Just about every physician in this State is a crook. They do not care about their patients. They only care about how much money they can make from their patients. Every person that works for the Arizona Medical Board should be fired. Why must Arizona taxpayers be responsible to pay these individuals for protecting corrupt physicians? I suppose we must accept it the way it is. Each and every day Arizona people are needlessly killed by corrupt physicians. I was also a statistic myself. Thank God my wife found me totally unresponsive and called 911. After two days in ICU I am still breathing. I wish I knew the answer to this problem. Good old street justice is what needs to happen. Unfortunately that would only result in being locked up because we do not have the Arizona Medical Board to prevent it.

Arizona Medical Board inappropriate touching during exam

On June 6, 2019 during a well women's exam at Southwest Woman's Contemporary Care at 16611 S 40th St Suite 180 Holly Campbell came into the exam, asked me to lay down on the exam table and preceded to open my gown, she confirmed I had a mammogram in 2019. She then began what I thought was a breast exam but what she did was place my right breast between her hands and rub back and forth. After a time she did the same thing to left breast. After some time she hurriedly flip the chaperone light. Waited for the chaperone continued with the rest of the exam. It has taken me a few weeks to process what happed to me. I wanted to believe that I was mistaken.

Arizona Medical Board northern arizona pain institute

I self discharged from them and I think they're upset and refusing to forward my Medical Records to my New Pain Management. Desert Pain and Rehab. I'm also experiencing the same issue with Corizon Medical and N.A.P.I knows this this is why they're being so Vindictive to me, ever since I started with this Medical Practice they have done nothing but make me feel like a Drug Addict.I had 41 Surgeries on my Rt. Foot and Leg and it resulted in a Amputation and the Pain is still the same and since the Pain Management Co. does not deal in Methadone I decided to go back to my Old Pain Manager. Desert Pain and Rehab. And they are upset.

Arizona Medical Board not following standard of care pcp doctor daniel kessler

My husband started going to Dr Kessler in 2016. when he strted he gave him a list of procdures that he had done. On it was his last colonsocopy which was 2008. Startand of care is every 3-5 years. My husband was a maire for 26 yers and alwasy follow his doctors orders. Dr Kessler never recommened tht he get a colonsocpy. My husband now has sage four colon cancer. If he had had his colonscopy in 2016 it would of made a big difference. It is no terminal. Plus when we wentback to see Dr Kessler after being diagnosed with it Dr kessler said "well I don't do a rectal so not my fault". Plus My husmads psa was high so doubly Kessler should of been more watchfull of it.

Date of Incident: 5/28/2019
My father has been going to see Dr. Kahn for approximately 5 years now. Dr. Kahn has always been great, informational, patient, kind and trusting once you get past his scheduler (front office manager). We rarely miss appointments but we did have a death in our family that resulted in a missed appointment and since then our experience with Dr, Kahns office has been difficult.

I called to apologize and reschedule the appointment and was informed he needed specific scan before they could reschedule him. I understood but needed the medical facility contact information from her. She was so rude, inconsiderate and unprofessional. When I asked to speak to someone else she put me on hold for 20 minutes. Nevertheless, I finally was able to receive the contact information. We completed the scan and I followed up with Dr. Kahns office to reschedule and again the Office Manager harassed me for missing the original appointment. She threatened me stating that there is a policy for missed appointments and warning me that my father may possibly no longer be seen at this office. When I asked to speak to someone else I was told that there was no one else. At this point, I was concerned because my father is running out of his medication that prevents him from seizing. I asked if I could speak with the doctor and I was told that the doctor don't talk to patients over the phone. Reluctantly, I asked if they received the request to refill his prescription from his primary care doctor and I was told that if they received one then it was shredded.

I understand there are policies and procedures. I am concerned for my father and other patients that may be treated like this. There is no urgency, no empathy and very ill patients.

Arizona Medical Board dr. biren patel, md - northwest urology

I had an 11:30 followup appointment with Dr Patel at Northwest Urology. At 12:15 one of his staff entered my examining room to obtain a piece of equipment. I mentioned to her that my appointment was at 11:30 and that my time was valuable. Approximately 5 to 10 minutes after that his assistant came in to the room to provide me with the results of a scan that I had done. I also mentioned to her that my time was as valuable as theirs and wasn't happy about the delay. At no time was I verbally loud or abusive to these people. When Dr. Patel finally came into the examining room he started reading me the riot act telling me to never speak to his staff in that manner. He said that he has a busy practice and this is the way they operate. Appointments are just estimated times when the patient will get to see the doctor. He said if this wasn't acceptable to me then he would not be able to have me as a patient. I said to the doctor "we are done here. I will find a new urologist" and I left the building. I've never had this kind of experience with a physician and I was taken aback. I think Dr. Patel is arrogant, unprofessional and just downright rude. I would never recommend this guy to anyone.

Arizona Medical Board medical care

I contacted my doctors office in the early afternoon today requesting to speak with my medical provider for follow-up care as what he was treating me for has not been resolved. I am severely immuno-compromised and told them I was only beginning to feel better starting 4 days ago on the Ciprofoxacin. I was told this nurse practitioner who treated me and prescribed medication for pneumonia and a bacterial infection in my stomach was busy with patients but if I waited for their call they promised he'd call me back. I did not receive a call. I have been very sick with ear-ache, stomach nausea, chest malaise, and sore throat. I called their office after 6 to see if/when they would be calling because my health is fragile. The answering service refused to contact anyone in the office on my behalf "because it only regarded prescription refill." Later, the service person said she spoke with the on-call nurse Julie who told them she was not going to talk to me, she refused to even discuss my condition or symptoms, and further said if I did not want to call in the morning I should go to an Urgent Care (at great expense when their office is free), or Hospital (even more expensive). Earlier this afternoon during business hours I stressed to the office person that I was very sick and needed to discuss treatment and my illness. She said the person could not speak at that time because he was busy with patients. The answering service was hostile to me as a deaf person, claiming I was yelling when I was not. I am upset, yes, but I was not yelling. I cannot hear and had to put them on speaker to amplify her voice better. The answering service said she "was not going to contact the office for any reason because this had to do with medication and I could call back in the morning." Given my fragility medically I do not think the office response of neglecting to contact me (#1) during business hours, and neglecting to followup with me (#2) after hours to communicate the issue to the doctor was appropriate. I think their behavior with an immuno-compromised person was neglectful to a gross extent.

Dr. Glazer,

I contacted your office in the afternoon today (05/21/2019) requesting to speak with the nurse practitioner (male), the medical provider who saw me because you were not at the hospital the day I sought care for illness after returning from an airline trip through Denver to Pennsylvania. He diagnosed me with pneumonia and a bacterial infection (traveler’s diarrhea) in my stomach.

I told the office person that while improved with medication, my illness has not been resolved. I told her I was concerned at ending medication before I believe treatment has been successful. I told her I had been without my morning dose and was feeling very ill. I mentioned believing I needed 2-3 more days of Ciprofloxacin but needed to speak with the provider to see what his input was.

As I said to her, and to you when we met, I am severely immuno-compromised. I told the office person that while it was working on me, it was slower healing me than he thought. He had given me a 7 day dose. I had just two days before ended having diarrhea and coughing up phlegm. The office person told me that the nurse practitioner who treated me and prescribed medication was busy with a patient. She indicated he'd call me back after he saw the patient.

I did not receive a call.

In one day I have been regressing. I have an earache, stomach nausea, chest heaviness, and a very sore throat. I took a very hot bath because my body aches were bad. I fell asleep with my phone by my side waiting for his call.
I called your office after 6 to see if/when they would be calling because my health is fragile. The person mentioned the “on call doctor” being an option. Then the answering service person told me she refused to contact anyone in the office on my behalf "because it only regarded prescription refill" and she was not going to get fired by doing it.
During a call-back, the answering service person said to me that she spoke with the on-call nurse Julie who told them she was not going to talk to me. The answering service said the nurse refused to even discuss my condition or symptoms, and further said if I did not want to call in the morning I should go to an Urgent Care ($65+), or Hospital ($150).

When I continued to ask for medical care to discuss my symptoms with the on-call person, the answering service got hostile to me as a deaf person, claiming I was yelling when I was not. I am upset, yes, but I was not yelling. She was trying to divert attention from what I was asking for, medical care, with a manufactured conflict which was very insulting. I cannot hear and had to put the call on speaker to amplify her voice better. Claiming I was hostile because I was asking to speak with a medical person about my symptoms, especially since she was denying me that care (something she first said was available but later said was not) constitutes medical neglect and I said so.
The answering service said she "was not going to contact the office for any reason because this had to do with medication and I could call back in the morning."

Given my fragility medically I do not think the office response of neglecting to contact me (#1) during business hours, followed by a second neglect with refusal to pass me through to a medical person to discuss my symptoms (#2) after hours was appropriate.

This behavior is medical neglect to a gross extent.

I have removed you, Dr. John Glazer, from my primary care. While this had nothing to do with you, it has everything to do with an extremely dysfunctional medical office which clearly does not feel it important to value each patient. No person should have to wait until it is convenient for your office to call, particularly not when in distress with illness and needing medical care. Being my primary care physician means not being “too busy” nor pushing me off with much higher costs on Urgent Care or a Hospital.

I do not trust your office and want nothing to do with a place that would treat me like this.

Do not contact me in any manner: not by phone, mail, email, fax, or in any other way or I will consider your contact as harassment.

I have since contacted a local practice and the person prescribed 5 additional days on the Cipro. I have had to pay out of pocket for this service. Had the doctors office fit me in, or told me I needed to go elsewhere BEFORE NIGHT when all there was available were hospitals at great expense which are packed, or urgent cares at significant expense which are packed, I could have gone in my local area to a clinic, to a local NP, or to the person I ended up seeing hours before I was seen and treated. I have now to wait until the morning to pick up my prescription, meaning I've been off the regimen for 24 hours. This is not okay given my immune system issues and the severity of my diagnosis.

Arizona Medical Board office visit with front desk and office staff in the room

I visited Dr Tarazi (April 11th ) and had a consult where he gave me treatment plans after we tried a less evasive approach. They made me make another appointment so that I would pay the $50 office visit (again) just to discuss this again. The 1st visit the front desk lady was in the room and the 2nd visit (April 29th) some other plain clothed lady (office girl) was in the room and she accused me of recording the visit. I told her I had no reason to record but thats a weird question if you are following correct physician practices. I was holding my phones but that was because i was texting my husband to let him know if I was having the procedure since I had made the appointment just for a 2nd consult. because they told me that was what I had to do. I was not able to talk with the doctor to let him know I was still in pain and wanted to go the procedure route. I gave the office a hard time that they wanted collect another $50 just so that they could bill another office visit. I wanted to talk to the doctor on the phone so that my next visit was actually the procedure. I had left 2 message prior to this and her never got back to me. Dr Tarazi is all about the money and NOT about patient care. I received a letter today that they were releasing me as a patient. Its bizarre that someone would accuse you of recording the visit if they were doing everything they way they should be. I don't think its appropriate to have office staff in the room and no MA.
