This is a formal complaint regarding UMR’s failure to provide my EOB despite multiple requests.

I am a former member requesting records for services rendered in Septenber 2023.
I have sent multiple fax and mail requests with no response.

This request is being made under HIPAA (45 CFR §164.524), which requires a response within 30 days.

If I do not receive the requested EOB within 5 business days, I will escalate this matter to the California Department of Managed Health Care and the U.S. Department of Health and Human Services (HHS OCR).

Please escalate this case immediately.
