On 10/24/2025, I completed a DOT medical examination at Concentra Urgent Care, 511 Busse Rd, Elk Grove Village, IL 60007, with a certified medical examiner listed in the FMCSA National Registry.
The medical examiner made a documentation error on my Medical Examiner’s Certificate by failing to indicate that I wear corrective lenses, even though this information was disclosed and evaluated during the examination.
Due to this error, my Illinois CDL was placed on medical hold, which caused me to be unable to work from December 12 through December 19.
I contacted the clinic multiple times requesting correction of the error; however, the issue was not resolved. As a result, I was forced to redo the DOT medical examination on December 18 in order to correct the documentation and resolve the CDL hold.
The error has since been corrected with a new medical certificate, but the delay caused unnecessary loss of income and hardship.
Claimed loss: Caused frustration and prevented me from working between December 12 to December 19.
Desired outcome: I request reimbursement for the week I lost due to this issue.
