I had the same problem. To fix it just call that 800 number on your statement (800-793-2818). After the LIVWEL* there will be an 8 digit number, that's the account number. Advise them that you don't want the service and don't know what it is. The Customer Service rep can only cancel the account and give you back 2 credits so you'll need to ask for a supervisor after that.
I just got off the phone with them, they were very nice about it. Apparently we got signed up for this when ordering flowers and signing up for a 15% refund, it was apparently a 30 free membership to something but we never got an email about canceling it, never got anything form them in the mail and had no idea we had even signed up for anything.
