Dear Old Mutual Complaints Department,
I wish to lodge a formal dispute regarding a policy that I allegedly concluded telephonically on 15 May 2026.
I have never received any policy documents, policy schedule, terms and conditions, confirmation SMS, policy number, or reference number. As a result, I have been unable to verify the existence and details of the policy.
Today, 30 June 2026, I noticed a debit order on my bank account which I believe may relate to this policy. However, because I was never provided with any policy information, I am unable to confirm whether this deduction is in fact for the policy discussed during the telephone call.
I therefore request the following:
A copy of the full voice recording of the telephone conversation held on 15 May 2026.
Confirmation that the debit order on my bank account relates to this policy.
My policy number and copies of all policy documents, including the policy schedule and terms and conditions.
Proof of when, how, and to which email address or cellphone number these documents were allegedly sent.
Immediate cancellation of the policy with no further premium deductions.
A full refund of all premium deducted,as I was never provided with the policy documentation or confirmation necessary to verify and review the policy.
Please register this email as a formal complaint and provide me with a complaint reference number together with written confirmation that my complaint is being investigated.
I would appreciate your written response within your complaint resolution timeframe. Should this matter not be resolved satisfactorily, I reserve the right to escalate it to the National Financial Ombud Scheme South Africa.
