Mutual of Omaha review: Payment not authorized

I applied for short-term disability through my employer. The case manager handling my claim denied my claim because he said that the never received my medical records. My doctor emailed me and him a copy of the service dates (which is what he said was needed). I asked him if I could appeal the decision and he said, "there's no appeal for this". Every time that he requested paperwork I and my doctor have submitted it to him or the department that he said it needed to be sent to. I have not received any benefits, and I have been on a leave of absence since 5/19/2026.
