  • Did they use fake testimonials or logos?

Attaching supporting documents
Upload:

• Screenshots of the sign-up/trial offer

• Email confirmations or account pages

• Screenshots of billing history or bank/credit card statements

• Chat logs or emails with customer support

Do not include sensitive personal data such as full credit card numbers, government IDs, or your home address.

Filing optional fields

• Claimed Loss: List the amount of money you lost. Example: “$19.95 unauthorized monthly subscription fee.”

• Desired Outcome: Be direct and specific. For example:

  • “Full refund and cancellation”

Review before submission
