Not wanting to give me uif19 form to apply for uif

I used to work for a company called Liberty for more than four years and last year I got dismissed because I had lost my mother and I asked them to give me time to mourn her and they didn't agree instead they chose to dismiss me so now I'm trying to apply for UIF and asked them to provide me with uif19 form to submit at department of Labour they all ignoring me my previous manager Natasha Arendse, Hr and the payroll department Dikese Nqae they don't want to give me the form to apply for UIF so please help me, I'm in desperate need

Desired outcome: For me to get the UI19 form so I can apply for UIF because currently not working and I have a 4 year old child depending on me

“Liberty Development” “ Fred Darwish” “ Latif Fazel” are bunch of crooks.

I purchased 2 condo units from “ Liberty Development” and to my surprise the builder uses the cheapest quality of material, from electrical to mechanical to finishes. Every single day there is an issue with my units that are tenanted and the builder is so careless that I have never heard of.
