Family Dental Care - Oak Lawn

My son has been going to Family Dental Care in Oak Lawn for the past 4 years. I never needed any visit summaries or school forms because he wasn't in school. Last week, my son turned 4 and got accepted into a private preschool. They gave me a week to provide any proof that he has seen a dentist before, or else they would give his seat to someone else without a refund. I called Family Dental Care 3 times and even scheduled future exams for my son. I told them I couldn't wait until the next appointment to get the proof of his past records, and asked if they could email them or if I could pick them up. The first 2 times, they said no. The third time, after explaining my situation, they were rude and sarcastic with me, and still didn't follow up on releasing his dental records. I don't owe them any money, and my son was seen this year, so his documents are up to date. I'm really disappointed with the customer service I've been getting from them for the past 2 weeks.

I was told on the phone that they did dental implants. I went to their office for dental assessments for a replacement of my tooth. They did a full mouth X-Ray and told me that it wouldn't be wise to get just one tooth implant. They suggested that I could get all my dental needs completed with the money I would spend on one tooth implant. I didn't like this decision because I wanted a permanent solution. The dentist came back and talked to me about a better choice - a partial. They took a molding of my tooth, but I was misled into thinking it was for a permanent tooth instead of a temporary one. I felt uncomfortable with their approach, but I trusted their professional advice and let them make the decision for me. They told me I had 2-3 weeks to change my mind because I needed gum treatments and deep cleanings before they could start any dental work, which would take about a month to complete. I left the facility and reconsidered. I realized that it wouldn't be the smartest choice at my age, so I called back after 2 days and cancelled everything. That's when they told me that my dental work had already been completed. I was shocked and said, "There must be a mistake!" I had only been to the office once for X-Rays and a tooth molding. They told me the work was done and they had put teeth in my mouth. I asked to speak to a supervisor because a big mistake had been made and I wanted to talk to someone who could explain it. No services had been provided to me at this point, so it was impossible. They said someone named "KeKe" would call me in a few days. I realized that things were going wrong.
