horizonoutlet / not what i though, need to stop

i took this card, the way i understood was there was a startup fee. i didn't thing that was every month, also you can't or its very time coming to get yo there store.also i'm trying to call e-mail, online to return this card and for them to STOP TAKING A MONTHY FEE.this has been going on for 2 or 3 moths.
please help me contact them or if you send this to them..
