Coast Dental Services Coast Dental in Wesley Chapel, FL, was to put caps on my teeth.

Coast Dental in Wesley Chapel, FL, were to put caps on all my teeth. They ground down the teeth too short and put caps on that we're too short; causing my gums to be severely inflamed. The teeth were too small and looked strange.

I subsequently went to another dentist who took scans and X-rays and showed the damage to the gums. A prosthodontist informed me the new teeth threw my bite off and was probably causing the severe neck pain which started when the caps were put on.

I have had to see a periodontist as well as a prosthodontist in order to correct the problem. All caps were removed, 2 teeth were extracted and 2 implants put in. I currently have temporary dentures which are used to correct my bite to where it should be. The periodontist had to laser my gums to make the teeth long enough to put the new caps on so we can save my teeth.

The procedure has been painful and frustrating. Not to mention the $90, 000.00 cost out of pocket to pay for Coast Dentals incompetence.

Desired outcome: Reimbursement for costs to correct problem. Warning to others…beware of companies that just want to sell you products and do procedures you don’t need.

Coast Dental Services Charged for services not rendered.

Dr. Chumpitaz of Deltona coast dental sent me a bill for services never rendered. Also he billed my insurance company for services not rendered. I called the office two days and they never got back to me. I need this to be resolved as soon as possible. The only work I had done was the initial visit where I received x-rays and an exam. That was all taken care of and I never went back to him. The bill said they performed periodontal scaling and gingival irrigation. I never had this done. Please negate this bill and tell them to return money given to my insurance company. My bill was 129.00. Humana plus was billed for 505.00. I would appreciate your prompt response. Thanks. Barry Koven, 2969 Greynolds St. Deltona Fl. 32738. [protected].

Desired outcome: Negate my bill for services not rendered. Also return the money he received from Humana.

I had exactly the same issue in Zephyrhills, FL. Both my wife and I were told we needed "deep" cleaning, irrigation, etc. We had those done less than a year before at another dental clinic. We told them only a basic cleaning, period. They didn't even polish our teeth. They have billed Humana for the deep cleaning and the claim was denied. This was in April of 2022. Yesterday, July 22, I received a bill for $349.00 with no explanation of what it is for.

All I want is this bill negated and if I get one for my wife the same.

Thanks, Robert C. Cathey Zephyrhills, FL

Coast Dental Services Refund

Hello, my name is Terry Johnson and I am seeking to get the refund I'm owed from Coast Dental. I went into Coast Dentals Lawrenceville Georgia office in February of 2021, I'm not sure of the exact date, with a toothache, they did a exam and the dentist told me I needed three teeth pulled. We scheduled an appointment for the extractions on February 8th. They told me that it would cost me $443.10 to have the teeth pulled and they would bill my dental insurance provider and refund me the difference. I paid them the $443.10, they did the procedure, I provided them with the dental insurance. I waited a couple weeks for my refund or a call from Coast Dental, I heard or got nothing. I called them and spoke with the office manager and she said it could take up to 90 days for the funds to go through the different channels before it can be released to me. I waited the 90 days and still nothing. I've called the office several times and it's always the same. All I want is the $348.48 that I'm owed. By the way, I forgot to mention that I called my dental insurance company and they told me that the only amount I should have paid was $94.62. Here we are on September 13, 2021 and I'm still waiting on my refund, it's been 7 months, can we please resolve this?

Coast Dental Services Overcharged due to disregard of hmo procedures

Beginning in December of 2020, I engaged Dr. Alfred Choi of Coast Dental in North Fort Myers to repair significant problems with my teeth. The first appointment involved Dr Choi looking at a severely decayed bottom tooth and wiggling the teeth around it. He proposed that we start that day and replace several teeth. He refused to give me a Proposed treatment plan as I declined service that day. I was waiting for an HMO plan from Humana to begin on January 1. The Office manager had already called Care Credit to have my credit line increased. I returned to the office on January 6, 2021 with my new Insurance card. I was informed at that time that full payment was expected at time of service and I would receive a refund from the insurance company. X-rays were taken and I received treatment plans for both upper and lower teeth. The following only refers to my upper teeth. That day I paid $1153.75 for Delux Partial Upper. Per HMO, the cost should have been $525. On February 11, 2021, I had 7 teeth pulled, Bone replacement graft and guided tissue regeneration, I paid $6, 293. for this procedure. the cost per HMO should have been $2, 018. All following procedures (implants) were paid for in full as they were not covered in the HMO. I have called the Office Manager and the Insurance company many times to rectify this problem. I maintain, that the Dentist, Dr. Alfred Choi owes me $4, 382 in overpayment for services. Finally, I am told by my Insurance Help Line that the $525 was to pay for a permanent partial. Aside from overcharging me initially, he intends to charge me again to replace this ill fitting temporary partial. I conclude, that Dr Choi and his office have committed theft by deception. In all, I am waiting for a refund of $4382 but expect a full refund to help cover the extra cost I will incur getting this job finished.

Desired outcome: Refund of entire payment

Coast Dental Services Cannot get refund

Paid for services at the Lake Miriam Square office in Lakeland Florida in November 2020. $1, 409.00
When I returned for the followup visit (to get cap put on)in December 2020, the dentist that I had seen had quit and not left the appropriate paperwork and had not ordered the cap.
A new dentist checked me and set up a followup visit
I returned in January 2021, and the cap was still not ready, the dentist that has seen me in December 2020 said said that I was not her patient and she would not work on me.
I went to a prior dentist and and an Endodontic specialist and they were horrified at the butcher job the original Coast dental dentist had preformed on me. they told me he has started a root cancel and left it unprotected and incomplete and a mess. It cost a lot but I had everything fixed properly.

I applied for a refund from Coast dental in February 2021. After many calls and emails, I was informed by the Lakeland office in May 2021, that the paperwork had been completed for the refund and sent to corporate for payment. I have called, faxed and emailed weekly since then and still have not received a refund.

Desired outcome: Refund $1,409.00

Coast Dental Services This Mableton Office Refused Treatment on a service I already paid because it was an different Clinician.

Hello Coast Dental,
Good morning hope you're enjoying your day so far. I (Gregory Parker DOB 4/30/80), made an appointment Sunday May 23, 2021 Online with the Coast Dental for the clinic in Mableton GA.

Chief Complaint: I have an broken Tooth that needs service or pulled. I also want to complete the service I already paid ($700 Plus) to put in my fake tooth into my mouth

I spoke with the Mableton office a couple times, they needed update insurance information etc... Never once converse I had an outstanding balance, but please remember this statement. 5/26/21 I arrive at 11:49am and was directed to the manager counter, Dawn Polk communicated I have an outstanding balance, and before moving forward I would have to pay. I asked to just finish the service I already paid for, she then proceed to walk and speak with the Clinician and upon arriving back she stated "He's not going to do it because he won't paid, the doctor who started the service is no longer here". I'm a Manager in UC and you NEVER communicate to a patient, the doctor refused treatment do to not getting paid. I asked Dawn Polk (Senior Office Manager) to put it in writing of what she told me, then she refused to do so. Ms. Polk then lied to my face and stated she didn't say that, which that's an total insult. It was a young lady at the front office the entire time that heard everything. I'm not trying to get her into trouble, but after Dawn left to speak with the Clinician the FO lady looked at me and said I heard her as well. I have never been treated like this in my entire career. I'm sure she meant to say this Provider won't receive credit since the other Provider already started, but that has nothing to do with the patient. Dawn is setting an bad example in front of her staff members by communicating to patients their refusing treatment because the doctor won't get paid. I accept to take an lie detector test 100% regarding this entire situation.

Please excuse any grammar errors typing through my phone.

Greg Parker [protected]

Desired outcome: To complete the job I already paid for at another center/ Give me full reimbursement including what the insurance paid.

Coast Dental Services Refund not received

I went to get my teeth cleaned on December 11, 2020. I paid $154 at checkout. After receiving my insurance statement, it appeared that they were paid twice for an item. This left an overpayment of $94.00. I was told I was correct, and they would refund my money in 4 to 6 weeks. I waited 5 weeks and called to check the status of my refund. Was told she would check on it on March 4, 2021. I called a week later and was told the same thing and she was sorry for the delay. I called repeatedly to no avail. It is now May 7, and I still have not received my refund.
All I want is for them to refund my $94.00.

Desired outcome: Refund my money immediately.

Coast Dental Services Root canal and crown experience

Re: Dr George Van Etta-Coast Dental St Augustine FL

My first appointment with Dr. Van Etta was on 3/3/2021. I had a full set of x-rays, a cleaning, four lower teeth were prepped for crowns, and temporary crowns were put on my four lower teeth. During the visit, Dr. Van Etta said I may need a root canal on tooth 25. Later in the visit, Dr. Van Etta said I did not need a root canal on tooth 25 and commented on my large tori. In addition to my Delta Dental HMO coverage, I paid in full $2,131.99 for the services described above.

I returned to the office about a week later to have permanent crowns put on my teeth. During this visit, Dr. Van Etta put two of the four permanent crowns on my teeth. He stated tooth 25 would need a root canal so he couldn't put the permanent crown on that tooth or the tooth next to it. I shared I would be having surgery on 4/16/21 and would need to have the root canal at least two weeks before of two weeks after surgery.

The temporary crown on tooth 25 came off the next day. I went back to the dentist the following day and they put the temporary crown back on my tooth. The temporary crown fell of again the following day. I spoke with Dr. Van Etta's office requesting an appointment and letting them know my concern about any possible infection and my upcoming surgery. They said they would need to speak with the doctor before scheduling. There was no response by mid morning, so I called the call center and shared my concerns. They scheduled an appointment for 11 am that day. Dr. Van Etta's office then called and said the doctor would be doing a root canal at my 11 am appointment.

Dr. Van Etta began the root canal. He hit a nerve twice and I shared it was very painful. His response was, "That's good. It means I found the nerve." I was not given anything to relieve my pain. Then, Dr. Van Etta said, "I can't get this, you need to see an Endodontist. He then told me not to come back there until I saw the Endodontists. This was a very painful experience. I was left wondering why he didn't know the complexity of this situation before he started a procedure he couldn't finish when he had xrays and was aware of my tori prior to the procedure.

I believe Dr. Van Etta did not provide me with adequate dental services and was not prudent in his practices.

Desired outcome: Refund for crown made for tooth 25 and the tooth next to it

Coast Dental Services Being charged for services

This is the first time ever for me to write a complaint letter, but I feel compelled to do so due to so many problems encountered at this coast dental facility at 2500 w lake mary blvd suite 106, lake mary, fl 32746. Most of the issues have to do with the crowns that have been made for me and coming off.

After having to return to the office over the past 2 years numerous times (at least 5-6) for an ill-fitting crown and at least 3 different labs to get the right fit on #14, it was frustrating and inconvenient. On one of those visits, dr. Arias had the dental tech make an impression with the sticky, gooey material and unbeknown to either of us, my crown on tooth #18 came off in the material. It wasn't until later that evening that I even noticed it missing and of course the office had already closed. The following morning, I called the office and I was able to speak with the same dental tech and I told her what happened and asked her if she could check the mold, she did and she was able to recover it. She indicated that I was fortunate because she normally sends out the molds before leaving the office at night... End of shift. I was able to get an appointment to have it recemented and thought all is well. Then early this year, #14 comes off again, but this time a dr. Miller, the current dds (dr. Arias is no longer at this location) was able to recement it back, using a different cement and a blue light led device and so far so good. All of these re-re-re visits were not charged and the office manager, ashley was very nice and accommodating.

Now is the issue of the crown #18... The same one that came out during the taking out impression mold stated above. I called the office and spoke with ashley and explained the problem and she made an appointment for dr. Miller to see me. Because I didn't want to take a chance in losing it, I manually placed it back in my mouth and it stayed nice and snug. After an xray, he examined me, attempted to remove the crown with his fingers and it didn't come off. He then tried using a scaling device and still no luck, then stated "come back in if and when it comes off again", and that was it.

Not more than three days later and after I finished eating a thai noodle dish, I sensed that the crown was no longer in place! Both my wife and I looked frantically through the leftovers, the garbage bag, and floor with no crown. Then my worse fear hit that I may have swallowed it! Of course, the only way to verify that it was in my digestive tract was to go through my stools every day... Not pleasant at all. After two days of no luck and researching on line, I went to the local er, lake mary advent health er. I was examined and had an abdominal x ray taken and it showed that the crown was in there, located right at the cecum/ilium. I was advised to continue checking my stools and return if I haven't passed it within 3 days. Three days passed and still no luck so I returned to have more x rays taken and the crown had not moved from the last spot. The er dr. Stated that she would have to call my gi dr. For further treatment and instructions. I had also called ashley at my coast location and she indicated that she would inform dr. Miller and just wait until the crown was retrieved.

A day or so later, I got a call from my gi dr. Stating that he wanted me to get another x ray (this will be the 3rd) if it hadn't passed in 3 days and take it from there. By the time the results came back to his office, lo and behold I had finally passed the crown! I called ashley again at the office and got an appointment for me to see dr. Miller. At that appointment, I was advised that I was going to be charged for another mouth x ray, exam and reattach the crown! Really?

I don't believe that I should have to pay again for this procedure when it's no fault of my own. The numerous times and issues with dr. Arias... The times of repeat visits... Inferior cement/techniques used (imo)…... The almost 2 weeks of daily checking of my stools... The er visits... And I truly believe that dr. Miller could have taken off the crown using forceps or other methods which would have not resulted in my swallowing the crown. All i'm asking for is the reattachment without me being charged again.

Desired outcome: No charge for repeated services that have already been paid

Coast Dental Services Billing

I visited the NSB office on November 13th. I was previously at a different dentist in March 2020. The unfortunately did not accept my insurance anymore. When I made my appt I told them I have been and had X-rays they told me ok and where. I let them know. When I went for my appt they took MORE and additional x-rays. When I questioned they said I will find out later. Then a dentist or someone came in and poked at my gums. Mainly 2-3s, maybe two 4s nothing higher. I was informed I needed an intermediate deep cleaning and it wouldn't be covered by insurance. I said I just wanted to normal cleaning and they refused to. I was told it was just like going into the doctors and them finding cancer and then not informing me and letting me live with cancer. Needless to say I walked out. I received a bill for $112 that my insurance didn't pay because they already paid for that service back in March. I called my insurance and explained the situation and they said they would pay it this one time. Then I received another bill for $14. My insurance only paid $98. I called my insurance and they informed me the dentist did not submit on the bitewings. If they wanted the additional funds they would have to resubmit. I have called and left messages and even email Coast Dental and no one has called me back. I called the office to ask them to resubmit and they wouldn't take my call because they are having a "high call volume". The bill is going to be paid if they talk to me and resubmit.

Please note this was also during the pandemic where people do not have the additional funds to pay for things. I was doing the right thing and getting my teeth cleaned.

Desired outcome: Resubmit to insurance or delete the bill on my invoice

Coast Dental Services Payment not returned

I feel that I was pressured into applying for credit for a 'better' bridge. I was approved and the bridge they offered amounted to almost the full amount of the $3000 loan. The month I was to get the work done I was ill. The next month, they were shut down due to the virus, and then my appointment was postponed several times due to no doctor being in the office. I asked to be transferred to another office to get the work done but was denied. I had to get the work done elsewhere and asked for a refund. I was first told that the office I went to would complete the return to the credit agency, then was told it would be done through the corporate office. I have made numerous attempts to get this rectified but still to this day, they have yet to contact me.

Desired outcome: A full refund of the money they receive for services never rendered.

Coast Dental Services Overall service - ortho and dental

I've been using this office for over two years because I needed a new dentist. While I was there I was told about their orthodontics and thought a consult would be good. I went for the consult, told them I wanted Invisalign, not only did I not get it, I wasn't given the option, so I settled on clear brackets. I'm paying out of pocket for everything and was told my treatment should have been 19-24 months. Since then, the office I go to in Port Charlotte has had 3 different orthodontists in this time. All of which did not communicate with each other as I wasn't using my elastics as they wrote in their records, my appointments keep getting canceled and rescheduled, one happened an HOUR before the appointment, never have any basic grey colors for my teeth. I'm still wearing braces after 24 months. With Covid we had to pay an extra $10 fee for their PPE... over all experience is awful. I haven't switched because I'd have to pay more out of pocket when it should have been done right the first time. I waited over an hour for my previous appointment which made me late for work.

As for dental, they breath down your neck and watch you write a review on google about their services.

If I could give this office a negative rating, I would. Cannot wait to be done with this bs and sad excuse for a practice.

Desired outcome: An apology, permanent retainers and a whiting at no cost for their incompetence

I want a refund do that he can go get the care we paid for and so that he does not suffer irreparable damage from the lack of dental care.

Coast Dental Services Dental procedures not performed

My daughter, Korri Barnes, has been a patient of Coast Dental in Sarasota and Bradenton, FL for 10 years and we have paid Coast Dental over $10, 000.00. Coast Dental - Sarasota Crossings on Fruitville Road - is the rudest, most unprofessional dental office in Florida. We have been trying to get her retainers remade for 6 months now. This is unacceptable. I called the first week in August 2020 to get an appt. The price agreed upon was $200. We had to wait 2 months for an appt. They called us in October, the day of the appt. to tell us they had to reschedule because the can not take impressions that late in the day. Now they set the appt time not us, yet it was for the wrong time. We had to wait 2 months to get another appt. We show up and they refuse to make the impressions until she has had her teeth cleaned. They cleaned her teeth and now had to wait 2 more months for an appt. Here to find out they made the appt for late in the day again so we can't get everything done today and will have to go back again. They are refusing to honor their agreed upon price. Now they want $600 instead of the agreed upon price of $200. I spoke with the orthodontics supervisor and she was extremely rude to me. We have been a loyal customer of Coast Dental for 10 years and this is how we are treated.

Desired outcome: We want the retainers made at $200 like agreed on.

We have been loyal customers of Coast Dental for 10 years and do not appreciate being treated like crap. My daughter has been trying to get her retainers remade for 6 months now. The agreed upon price in August was $200. You now how important it is to wear your retainers. Why are you making us wait 6 months? And you are now telling us the price will be $600 instead of $200. And rude people you are. I think you would treat complete strangers better than you are treating us.

Coast Dental Services services not complete

I am still trying to get my teeth fixed since January . It is usually a simple procedure but this bridge work has turned into a nightmare. My insurance and myself have paid in full but the work hasn't been completed. Biggest issue is that I have seen 2 different doctors and one doesn't like what the other started and now they want to charge more money . I feel that they have overcharged and are not a company that will do the right thing. They have referred me to another dental facility for a consult that they started . This shows that they have no confidence in what they did and wanted a second opinion. The problem with that was they did not want to pay and wanted me to pay out of pocket.

Coast Dental Services Refund

Re: Nicholas Burke

I have been speaking with Monique at the location in Gainesville, Florida off of 34th street office. A refund of $270.00 was supposed to be credited to my bank account. This all started in December of 2019 when I was told the refund would be sent. Instead it is 8 months later still waiting for the rest of the refund to be deposited into bank account. I have received increments of the refund one for $50 and the other for $93.00 balance still owed to my credit card is $127.00. I have never had a doctors office take this long to refund a patient. I have also left a message for the refund department at the corporate location. Please call me at your earliest convenience so we can get his resolved as soon as possible. [protected]
