Arizona Medical Board review: Post hospital exam not done

My medical care is under the VA. I was released from Banner Hospital on July 14, 2025, with instructions to have cardiac exam, since one of the diagnoses was acute heart failure with body swelling. After arriving home, I stopped at Pima Heart, Sierra Vista, and was advised no appointments were being made until after July 28 due to changing their system. Appointment was made for July 30 @ 9:45 am. A message from Sierra Vista office to their Tuscon office was made to see if an appointment could be made sooner in Tuscon. I waited until 7/15/2025, without a response and then had my wife call the Tuscon office and left a voice message for Dr. Meneses medical team for a call back to let me know if I should resume medication I was taken off of at Banner. That call lasted 25 minutes and 4 seconds from being on hold and leaving message. No response. On 7/18/25 my wife called 3 times with a busy signal. The 4th time she waited until it was answered, again to see if we could clarify medication. Receptionist (?) said she would transfer to medical team. Call lasted 54 minutes 42 seconds on hold between receptionist and transfer. There was no pickup from medical team. Today, 7/30/25 I kept the appointment and was told by Dr. Menenses, who has been my doctor for almost 5 years, that since his staff did not like my questions and attitude, that he was firing me as a patient and no exam would be done and he would not comment on medication status. He also said his legal team would be sending me a letter. This was after I was weighed, blood pressure taken and ekg done for my heart. He also informed me that his office did not make return telephone calls and that the last time they had a record of a telephone call from me was in June. He said his system showed no calls were received in July. Since these calls are on my wife's call logs, I don't know why they are not on his. So, I still don't know if being off medication prescribed by Dr. Menenses is detrimental to my heart condition.

Desired outcome: An apology for letting staff members dictate that a patient should not treated for medical care when that patient is nonviolent and for firing me as a patient without regard for my physical wellbeing.
