Dear Practice Manager,
I am writing to formally lodge a complaint regarding the substandard care, excessive delays, and highly unprofessional conduct I experienced during my appointment today with Dr. Jari Rajas. This was my second visit to your clinic, and the treatment I received fell completely short of acceptable medical and professional standards.
I am requesting an immediate review of the following events:
• Staff Misconduct and Excessive Delays: My appointment was delayed by over an hour. During this entire time, the clinical and administrative assistants ignored the delay and were loudly discussing Facebook and other social media platforms. This demonstrates a severe lack of professionalism and a total disregard for patient time.
• Procedural Failure and Lack of Informed Consent: When Dr. Rajas finally attempted to administer the injection, he broke two separate needles. Neither Dr. Rajas nor the assisting staff informed me of the risks, explained what went wrong, or warned me about the pain involved before or during the procedure.
• Patient Abandonment: After failing to complete the procedure, Dr. Rajas abruptly walked out of the room without speaking to me. He merely instructed his staff to have me reschedule and left. He provided no explanation, no evaluation of my condition after breaking the needles, and no opportunity for me to ask questions.
The combination of a distracted staff, failed procedural execution, and a complete lack of communication from the physician is entirely unacceptable. This experience has entirely compromised my trust in your facility.
Please consider this a formal request for a supervisor or manager to contact me directly to discuss how this matter will be handled. Furthermore, I am requesting a full, unedited copy of my medical records from today’s visit, specifically detailing how the broken needles and the incomplete procedure were documented.
I expect a prompt response regarding this matter.
