All,

I submitted my application for unemployment benefits, on Oct 2, 2023.

I have provided the correct physical address 730 Keppel Drive Newport News Virginia, which was changed in your system 7302 Keppel Drive Newport News Virginia, WITH BEING DELIVERED ON TIME.

I have provided:

1) Drivers License

2) Social Security Card,

3). Passport

4) Birth Certificate Born: (Yvonne M. Lewis) My Birthday was changed and has yet to be corrected, I was born On April 25, 1957. Married on 30 April 1980, Ronald Cook I took his name while in the Military. Provided Marriage License

5) My DD214 reflects this and so does the raise seal document provided by North Carolina.

6) This followed up with you changing my name being change back to Yvonne Lewis and after additional visits to the local office in Hampton (5) it was changed back to Cook.

7) I have faxed all these documents both from the Hampton VEC and personally from KINKO/FEDEX.

8) I have called your office for almost 45 days while also visiting the Hampton VEC offices over 5 times.

9) I have provided my Citizenship documents.

I cannot log in and just received my PIN.

Please make the appropriate correction, to my birthday APRIL 25, 1957, RESET MY LOGIN ACCOUNT AND MOST OF ALL BACK DATE MY PAYMENT, SINCE COULD NOT LOGIN WEEKLY.

I HAVE YET TO RECEIVE ANY PAYMENT. I WAS GIVE A COPY OF ALL THE ANNOTATED NOTES YOU HAVE IN THE SYSTEM
