These people have billed me for $741.75 for insurance premium s covering 3 months. I have no idea who they are and they refused to supply any information unless I give them personal information even though I supplied them the Billing control number and Payment ID listed on the bill. Based on these facts I must consider this bill a scam. as a legitimate organization would have enough information to identify their claim.

Desired outcome: do not contact me again and withdraw their claimed payment due
