  • Did you miss a filing deadline?

  • Did the delay weaken your case?

  • Were you financially affected or emotionally stressed?

• Attempts to resolve:

  • Did you try contacting different departments or a managing attorney?

  • Did you file a bar complaint or ask to transfer your case elsewhere?

Attaching supporting documents
Upload:

• Intake forms, retainer agreements, or signed contracts

• Email exchanges or chat logs

• Notes on phone calls (dates, who you spoke with, what was said)

• Any correspondence showing delays or dismissals

Do not include full personal details (SSNs, case numbers, etc.) unless redacted.

Filing optional fields

• Claimed Loss: Be specific. Example: “Lost potential settlement of $25,000 due to missed deadline and case dismissal.”

• Desired Outcome: Clearly state what you want. For example:

  • “Formal case review and reinstatement”

  • “Written explanation and apology”

  • “Referral to a competent attorney and reimbursement for damages caused”

Review before submission
Carefully review your complaint:

• Make sure it’s well-organized and factual

• Check for correct dates, names, and amounts
