1) I'm requesting that Elevate Recoveries provide me with proof that there was indeed a contractual agreement between myself and Elevate Recoveries signed and dated with both my signature and also Elevate Recoveries signature on it, stating that I agreed to pay this fraudulent debt to Elevate Recoveries. 2) I'm requesting that Elevate Recoveries provide the beginning and ending of this inaccurate service that I rendered from Elevate Recoveries with my signature and again Elevate Recoveries/Agent signature on it. 3) I'm requesting that Elevate Recoveries provide me with documentation that they went about the CORRECTED LAWFUL way under the Fair Debt Collection Practices Act of 1977 of warning me about my RIGHTS to validation in writing or by mail within the CORRECTED initial TIME frame by law. 4) I'm requesting that Elevate Recoveries can show me Proven Documentation that their company practices are not showing fraudulent purchasing of individuals personal information.

This complaint regards a collection that you posted on my trans union credit report. The account number is XXXXXXX for Paragon Emergency Services, a company apparently you are trying to collect a debt for. This is a debt that I do not owe, I am filing disputes with Trans Union, the CFPB, and the Attorney Generals Office. Unless this debt is removed within 30 days I will continue my efforts with complaints/disputes with the above mentioned organizations. I do not owe this debt please remove it immediately. I am fully aware of my rights according to the Fair Credit Reporting Act, and will not rest until this debt is removed from my credit report.

My husband got onto his credit score anf seen that there was a drop in his credit score we called these people and noticed that he does not have a bill for them and called anf talked to the hospital and he has no money to pay them . I seem there are alot of fraud complaints about this business. Elevate recoveries llc we feel it's a fraud business as they could not get any answers from them. On top of that we called it again anf then the one girl proceeds to say that he has zero to pay and this back and fourtb and totally confused if someone can look into this would be greatly appreciated

I received a letter from Elevate Recoveries stating I need to pay $920.00 for services at St David's Medical Center Emergency Department in 2019. I have never been seen at this Emergency department and demand for this company to LOSE my contact information! Do NOT, I repeat Do NOT contact me again outside of the ComplaintsBoard.com website and I had better not find ANYTHING on my credit report from this fraudulent company!

Elevate Recoveries Received a call today from Elevate Recoveries saying I owed over $1000 from a medical bill from a physical from South Side Regional Hospital

Received a call today from Elevate Recoveries saying I owed over $1000 from a medical bill from a physical from South Side Regional Hospital from 2019. They also stated that insurance was provided (I have Virginia Medicade) so the hospital and the physical knows the rules to accepting such state insurance. They went on to tell me that was the balance left over, there cant be a balance because all of my medical is covered UNLESS he did a procedure that is not covered under my insurance which they are not allowed to do and I certainly didn't consent to any services not covered by my plan. They were rude , hung up on me and didn't want to address the issue after I told them what they needed to do since they admitted to having my insurance information. No resolve was done besides them taking my number off their call.list and insisting that my credit would be affected. I have never received a bill for these charges nor have I ever been made aware of these charges and it is 2022!

Was contacted a few weeks ago from this debt collector telling us it was a hospital bill from over 2 years ago. Was hounded into trying to set up a payment. I contacted the hospital and no such billing can be located. I am asking the debt collector verify this billing including that they have the correct *** and if not to please close the account and remove it from being on my credit report. It appears this date of service or patient was not actually me after all.

On 02/25/2022 a derogatory collection was placed on my credit report by Elevate Recoveries LLC in the amount of $1507. It shows that the account has been open since 10/23. And it's showing Paragon Emergency Services as the original debtor. I was incarcerated for the last 4 years, and I am unaware of this company and have never heard of it. I am unsure when this debt could have occurred, or what it could be for. I do believe they are in violation of the fair debt collection practices act. Please be aware That I have not been contacted by this company in writing, or by phone to notify me of this debt which is a violation of multiple laws and statutes. I demand that you cease and desist all collection activities, as it is uncollectable, as well as invalid. I will further research my options if this issue isn't resolved.

Elevate Recoveries but a collections on my credit report without sending any collection notifications. After disputing this via the credit agency I called Elevate and learned... They are trying to collect $156.56 for a visit on 1/6, which is over 5 yrs. ago, but only opened the collection 2/4/2022 The company they said to be doing the collection for is Code 3 ER, Frisco TX I am not able to confirm if I was ever a patient at the location because it is now permanently closed All of the Code 3 ER & Urgent Care locations are permanently closed & all phone number on the corporate site no longer work either This is a complete scam! I want the collection account closed and removed from my credit report immediately!

On 5/22 I had Universal Healthcare for a procedure at Northbay Hospital.Confirmed, my insurance paid in full for services rendered. (XXX) XXX-XXXX phone for billing of Northbay. 2/20, I received a letter of collections by a collection's agency for several thousand's of dollars.There were no dates of service on the letters. At the time, due to Covid, no one was answering phone calls at this agency.The agency reported to the credit agencies (prior to 30 days), to which I appealed. The credit agencies (Transunion and Equifax), sided with my appeal and withdrew the collections from my credit report.On 2/11, another collections agency has now, "purchased"this debt?There is no debt to purchase.Per California law, if the patient never received a bill, a patient cannot be sent to collections.However, the point is moot, as my insurance paid every single practitioner for the date of service on the attached document.I am terrified this is an elaborate attempt to extort a consumer.

I found Elevate Recoveries LLC on my credit report today. There's an alleged balance of $106 however, I've never been sent a bill for this amount from anyone anytime prior to 11/17, the date it was reported to the credit bureaus. I want this corrected, I can't pay something I don't know I owe and don't receive in writing!

Hi, I received a call today stating there was a ding on my credit report bc they just received a debt collection in November for an alleged hospital visit in May. I NEVER received a letter, although they claim they sent one to an address I haven't used in years. This does not sound legitimate. I researched and can find no validity to this claim. It seems VERY bizarre to me, that a bill from 2017 would just now reach collections and have never come to me previously. I want this removed from my credit rating and the alleged "debt" removed.

On 1/21 I informed Elevate Recoveries , LLC that the bill referenced was not valid and that there is no current bill/balance with the original creditor, *** Hannah informed that I could email proof. I forwarded a copy of all charges, paid over 1 year ago by *** that showed a $0.00 balance and requested immediate deletion. Additionally, the correspondence provided by Elevate Recoveries, LLC, dated 1/6 states "If you do not pay this account(s) within 45 days from the date of this letter or dispute the validity of the account(s) as described on the back of this letter, the account(s) may be reported to one or more credit reporting agencies." This collection account was reported 1/28, which is not 45-days from date of letter therefore, the incorrect/invalid account should not have been reported regardless. Elevate Recoveries refused to remove the account and stated "it's there until it's paid" which is an unfair practice that goes against the FDCPA.

I am a victim of identity- theft, I am writing to request that you block all accounts disputed as fraudulent on my credit reports with Transunion, Equifax, and Experian. This information does not relate to any transactions I have made. I did not give anyone authorization or consent to use my personal information. First Choice Emergency Room Spring Rayford (First Texas Hospital) never submitted a claim to my health provider back on 06/02, they sold the account to Cascade Capital Funding LLC. I did my investigation with insurance provider back in 2017 and either 2017, 2018 or 2019 this business never submitted the invoice to Blue Cross Blue shield of Minnesota. BCBS are in the process to send me by mail copies of all my statements as prove. First choice Emergency room closed their business permanently and sold all past dues to Elevate Recoveries LLC, this last Company put me in collection, I am in the process to buy a property and willing to lost my earnest money of $5,000.00 besides not able to get the property I want. I spoke with them and the only way to take me out of collection is paying the settlement amount $2,353.20 out of full amount of $3,922.00.

The reps was extremely rude and hung up on the phone several times on me. Collection paid 08/17 on a recorded line and asked to make sure the debt would not be sold. 08/27 debt sold. We were Harassed hung up on talk nasty to. I called the other company to confirm the account number and it was confirmed that it was the same account. The representive I was speaking too was being very rude and continued to overtalk me so I asked to speak to the manager when the supervisor got on the phone she stated I need to send proof that I was paid I asked her how long will the process take and she insisted on stating she didn't know when the department gets to it they will so then I asked to Confirm the company by asking for their website and she would not give me the website I asked to speak To her manager and she stated she is the owner of the company I asked several times about her website she would not inform me of her website and ended up hanging up on me because I was asking for their website.

very rude Collections agent. she stated the amount of debt and i do not have a signed and or notarized agreement / contract with your company. Collections agent calls more than 3 times a day no message ever left. the one time i do answer phone while at work the agent was very rude. stated the amount of this supposed medical debt of $214. when i stated i do not have a signed and or notarized agreement / contract with your company to conduct any business with you, she then proceeded to tell me that i have no idea what i am talking about and the debt is owed. Unless they can provide a signed and notarized contract agreement between myself and the company, and said contract has been signed and notarized by both myself and the company then this debt is void. I have never agreed to conduct my business with this company. This debt is to be cleared and fulfilled. Once again Collections agent stated that i have zero clue what i am talking about. i have been in this industry before and no need to be rude i am aware of what i am talking about.

Elevate Recoveries This company was more punitive than corrective

This company was more punitive than corrective. They submitted false information to the credit bureau. I continuously get phone calls from this company. They called my phone asking me for personal information, such as my name, address, social security number. Wanting personal Information on who I was. I asked them who they were because I wasn't going to share my personal information with them over the phone unless I knew who they were. The lady I spoke to was rude, obnoxious and told me that I needed to verify who I was before they would tell me why they was calling or give me any information. They should know who they're calling and let people know who they are and the purpose of their phone call. I then get a bill for 1,838.00 from this company. Come to find out this is a workmans compensation claim that should have been paid by workmans comp. It should have NEVER affected my credit. However, The next thing I know, my credit score went down 86 points cause Elevate Recoveries files against my credit. Had they taken the time out. They would have learned the facts. Instead Elevate Recoveries is being more punitive than corrective.

First of all i need an itemized billing for what it is after all thesese years iam not sure what they billing me for? They never called or contacted me. After all thesese years what were they doing? i need this shady/scam of a collection company with multiple SCAM ratings off----------i uploaded the letter they send me. Multiple people reported them A SCAM COMPANY online and reported to ComplaintsBoard.com. Please take necessary steps to get thyem off/i cannot share anything with this company as they do not pick up phone and is rated SCAM/HARASSING/RUDE company by people online. Elevate Recoveries Currently, Elevate Recoveries LLC, hereafter referred to as E

Currently, Elevate Recoveries LLC, hereafter referred to as E.R., has placed a collections claim on my credit report. The current claim was opened on November 3rd of 2021 according to my credit report. The collection itself is reported to be in relation to a visit to the hospital back in 2018. During this visit I paid for the services I received before I left the hospital. I asked if I owed anything more before leaving and was assured I did not. Several months later, I began receiving calls from a collections agency on behalf of Paragon Emergency Services LLC. I informed this agency that they were attempting to collect an imaginary debt over several calls. Eventually I stopped receiving calls and no claim was placed on my credit report. I have recently began receiving numerous calls while I am at my place of work and have been ignoring them as they are marked as spam. I then received a claim on my credit report of $1,624. I filed a dispute through Credit Karma to no avail. I researched the company that placed the claim and was disturbed by their track record of fraudulent claims. As the initial treatment and claim, which was removed and not pursued further, was over three years ago I do not have and paperwork in relation to it.

Elevate Recoveries I was awakened in the night feeling as if I was having a heart attack

I was awakened in the night feeling as if I was having a heart attack. My wife took me to the nearest Emergency Center, which was a small Emergency Center called First Texas Hospital. The first question we ask the was: do you accept and bill Medicare? She answered yes. I then saw the doctor and I ask him, if they accepted and billed Medicare and he said yes. After some initial testing, they said I was not in danger. Then they called an ambulance, and I was taken to Memorial Herrman Hospital in Humble. Next morning released to go home. We began receiving huge bills, for what they claimed was done on me at the cost of $9,961.00. I immediately contacted them and ask if they had billed Medicare? They said NO! I asked if they would please bill Medicare for this and then my insurance would pay the bill. This procedure went on for the next 6 months, of numerous phone calls to them, begging them to bill Medicare. I called Medicare to get help on this and they said they have to bill Medicare I cannot do this. Finally, after 6 months of begging and calling I quit completely trying to get them to bill. I have heard nothing more about this in last four and half years, until I received this bill from Elevate Recoveries. We are both 78 years old and do not have the funds to pay this bill at all. Can you help us resolve this issue?

Elevate Recoveries I am being harassed by this debt collection agency as a result of an erroneous debt that they claim I owe

I am being harassed by this debt collection agency as a result of an erroneous debt that they claim I owe. I have provided substantiating evidence multiple times over which verifies that I do not have any remaining balance and do not owe anything to Code3 ER as a result of an accident which I was not at-fault. I have also verified with my then-insurance provider, UnitedHealthcare that I do not owe anything, and have documentation from the at-fault party's insurance provider stating they paid the sum of remaining balances to Code3 ER. I also have had conversations with billing reps from Code3 ER where they informed me there were MANY accounting errors as a result of a third party billing transition, and this erroneous debt is most likely due to that. Regardless of providing this information to Elevate Recoveries, they continue to harass me and force me to re-explain all of this multiple times over. Each time, the Elevate agent assures me that my debt is resolved; and inevitably I end up hearing from them within the next few months. I am on the verge of filing criminal harassment complaints against this company for their mismanagement of my record and failure to acknowledge my well-documented history of investigating this issue. Anything the ComplaintsBoard.com can do to investigate and penalize Elevate Recoveries is most appreciated. I also filed a separate ComplaintsBoard.com complaint against Code3 ER which is an ongoing investigation and they failed to respond to the complaint.

After communicating with FIRST CHOICE BILLING DEPARTMENT, the original creditor, they allowed the minor's financial responsible guardian to assume all responsibilities of this account in August . They requested she provide them with the signed documents, DL and SS card which was done. Once all requested documentation was received, they transferred the account to the mothers name and updated the account with all her information. After the transferring of the account, all my information was removed from this account and all further communication that was being had was between the original creditor and the minor's mother. The original creditor made no attempts to contact me by writing nor phone about any unpaid debt after August . I have no contract or financial responsibilities with Elevate Recoveries LLC and no attempts were made to contact me prior to listing this erroneous information on my credit report which, I believe they are in violation of the Fair Debt Collection Practices Act. I've tried contacting the the original creditor, FIRST TEXAS HOSPITAL but have found out that they are permanently closed and the account holder is now deceased. I'm requesting this incorrect reporting be immediately removed from my credit report. I am not aware of any debt owed to either the original creditor or this company and this debt claim is false.

Elevate Recoveries Agency has wrong name, dob, date of service on medical bill

Agency has wrong name, dob, date of service on medical bill. Will not provide attending physicians or services rendered. Not my bill! Elevate Recoveries, LLC are attempting to recover a bill which is not mine. The bill is not in my name. They gave the wrong date of birth for me. 1925 indeed! I had no medical treatments completed on dates mentioned. They refuse to detail attending physician/s or services/procedures rendered. They offer to accept a lesser amount to end the harassment or will report me to credit agencies to ruin good credit. This same bogus bill was previously attempted to be collected by HRRG, a collection agency in FL, which just happens to offer the services of a Law Firm to "end the harassment " and "save your credit score". HRRG, incidentally, accepts payments through ICP HEALTHCARE SERVICES OF TEX at addresses in North Hollywood, CA or Cincinnati OH. That bill was subsequently dismissed when proof of services rendered could not be shown. Obviously that is when the bill was passed on to Elevate Recoveries, LLC located at 1930 N. Grand Ave., Sherman TX, 75090. It is unconscionable that companies such as these, given the current pandemic, prey upon everyday folk that meet their financial accountabilities and strive to maintain a good credit score. This is not my bill, and furthermore Elevate Recoveries, LLC know it. It also says something to me that their business sign is a temporary vinyl banner located in a strip mall beside Wally's Party Factory. Their letters and phone calls are meant to intimidate the honest and unsuspecting and frighten them into payment so as to avoid further legal issues and/or cost. Should you require any supporting documentation, please do not hesitate to ask. Thank you

Complaint against Elevate Recoveries & demand for their rectification of fraudulent demands/credit reporting. 2/21/22 - I rec'd the ONLY communication from Elevate/ any org. listing a First Texas Hosp. $11,955.00. 2/28/22 demanding pymt listed for 11/4 (6yrs ago) for a FTH. Letter gives 30 d to respond. Elevate ALREADY REPORTED to CREDIT BUREAU derogatory ON 2/28/22 bf letter rec'd that day. Only form of ANY communication on FTH by ANY organization including FTH! Elevate is in violation of FTAC, and multiple TX/US Laws. Aetna is now mentioned for further proof. Aetna tried to intercede and assist as outlined below. Aetna has on file- 1 communication from FTH over 5 yrs ago 1//4/17. Aetna response 1/11. FTH submitted blank paperwork. Submission was for a date 11/4 & lacking coding /documentation. Just $ Aetna responded to FTH stating NO FUNDS were owed on behalf of listed individual provided. FTH given correct process/ also sent a check in the amt of $1000.69 on unrelated individual in same correspondence. Add'l proof of communication w/ FTH. To date 6 years later, neither Elevate,FTH,others have attempted ANY form of comms w/ Aetna OR named individual until this letter. All FTH locations defunct. Elevate ltr says debts bought. Aetna PROVIDER relations along w/ listed party, made 2 attempts 2/28/22 to speak w/ Elevate to show proof/ ALL prior documentation. Elevate Rep -Regina ID 209 stopped responding but confirmed she heard us. She slammed phone. Second call attempted. A man answered. Refused name. 2attempts. He said Elevate & slammed phone. All items and actions listed above are on file/calls recorded by Aetna for verification. Correspondence btwn FTH/Aetna from 2017 on file. 1)Elevate has 7 DAYS to remove ALL reporting against listed party to ANY/ALL credit bureau/reporting agency/financial institution to prevent legal action. 2)Admission of error letter to individual $0 bal for file Cc: Legal Counsel. Aetna awaiting

1) They have communicated after my asking them to stop via verified letter. 2) They have made false statements and misrepresented themselves 3) They sent a letter to my parents address thus improperly sharing my information 4) They have attempted to collect a debt not owed as I have no record of this bill and they said that they had closed the account due to an inability to verify it I submitted a complaint against CACi for the same matter earlier this week. While their response was satisfactory, it is clear that it was not true as I continue to be contacted by Elevate Collections for the same alleged debt that CACi attempted to collect and then subsequently discharged according to their response to CFPB complaint number XXXXXX-XXXXXXX. CACi initially mailed a statement to my parent's address in November . When asked, the representative at Elevate said that they mailed a letter on the same date as CACi despite me having no record of a second separate letter. Upon hearing this, I realized that Elevate and CACi must be part of the same group and that he was talking about the same letter, but your representative assured me that that is not the case and that he has never heard of CACi. When pressed further, your representative said that the debt no longer belonged to the original medical group he had initially said that it did and that Cascade Capital Funding LLC was the new owner of the debt. On March 30, 2022, I called Cascade myself and asked the representative what their relationship to CACi and Elevate was. She candidly said "Oh- that's us! We're all the same!" Knowing this, it is clear that Cascade Capital, Elevate, and CACi are all engaging in deceitful and fraudulent collections practices. CACi assured me that the debt had been written off as of March 15, 2022 and that they would no longer contact me in large part because they could not produce sufficient proof to verify the debt, but it is clear that contact has resumed and they have lied

Elevate Recoveries is attempting to collect on a debt that I already paid to the original creditor, which I have proof of payment. Elevate Recoveries has wrongly opened a collection account on my credit report for a hospital visit and services I have already paid to the original creditor in full. On top of that, Elevate Recoveries is alleging that I owe more than the original amount owed for the services which is absolutely ridiculous. Immediately after receiving the first collection notice in the mail, I contacted their office via the customer support email located on their official website which they failed to respond to. I attempted two additional times to follow up on my dispute. It should not take more than one attempt to contact this business to receive a response, yet they showcased how low their quality standards are. This is not the first collection agency that has tried to collect on this same nonexistent debt, Elevate Recoveries is just the only agency incompetent enough to open an account on a disputed debt that has not been undoubtedly proven to be mine. Attached below is my attempts to dispute with Elevate Recoveries directly and an incorrect "verification of the debt" they sent long after my attempts to contact them. Please notice how Elevate Recoveries notes at the bottom of the email that I can dispute this new accusation within 30 days from 11/13 before they assume the debt is valid, but they have already opened the false account on my credit report within that timeframe, meaning they have already assumed the debt is valid. I have also included below the receipt of payment for the only debt I ever owed for the hospital visit. I attest that the amount shown in the receipt is all I ever owed and that LewisGale Medical Center is the only facility I received services from. I marked out my email and last four digits of my card because I do not need Elevate Recoveries harassing me more than they already have. Honestly, I feel that Elevate Recoveries owes me $790 for the aggravation they have caused me with this false collection. I feel sorry for the many people these scam artists (Elevate Recoveries) have successfully intimidated into paying a debt at a higher fee that has already been paid. It's unethical. Elevate Recoveries decided to try to scam the wrong person this time. I would shut their whole business down by myself if I could. I hope whichever representative of Elevate Recoveries has to deal with this complaint takes a moment to realize how inept their business practices are and genuinely tries to improve going forward.
