MyChart App - The Gastroenterologist, Hematology and Nutrition department scheduled (5) appointments through the Cleveland Clinic MyChart App for a in-person visit. The (5) appointments in question were not received through my assigned MyChart message board which resulted in the termination of care by my care provider at Cleveland Clinic. Communication had failed due to the MyChart App, no Phone Calls received, Text Message, Email, or message through MyChart App. Unaware of these appointments and the specific care needed due to my disability, my health and medical needs are now at risk of infection, kidney failure, heart complications and severe dehydration. Additional assistance was not offered or provided to retain a new care provider, and no assistance was offered by the Ombudsman Office.
Recommendation: Do not rely on the MyChart App, advise scheduler to contact you by phone
