Round Table Pizza review: cleanliness of facility

I am a local and have been eating at this round table almost my entire life. Yesterday when I ordered the salad bar and pizza, I couldn't help but look around and notice the appalling state of not only the restaurant, but the employees themselves. It was easy to see that the main counter and not been wiped down in sometime as it was covered in greasy fingerprints. The salad bar was the same. As I walked away looking for a table I could not find one that was clean. Multiple tables had dirty dishes on them, and the ones without dishes had clearly not been wiped down. As I ate I watch the busser clear all the dishes and not wipe the table. She cleared the dishes and went and sat out a booth where she had an infant.
The final straw, and ultimately why I'm writing this you, is when I went into the bathroom. It was very clear that this bathroom had not been cleaned anytime recently. I do not want to go into detail but as someone who's not offended by a somewhat dirty bathroom, this was the most disgusting bathroom I had ever seen at a restaurant. It resembled the dirty bathroom behind the gas station at a rest stop. I immediately had to leave the restaurant as I was so embarrassed.
On top of all this there must have been eight employees working. It was clear that these basic cleanliness items we're being overlooked. Please have someone look into the cleanliness of this restaurant as it must be a health hazard and a stain on the community.
