I am assisting an elderly friend who since 1/24/26 has had I think 5 er visits, 4 admissions (1 on observation status), was discharged to an snf rehab to after first admission.
It often seemed goal was discharge asap with disjointed coordination, lack of full evaluation of all body systems.
24 hours after 1 discharge to sng rehab, back to er: acute cholecystitis, surgery (poor surgical risk), another back to ER next day with sepsis-severe uti.
finally at the last admission, we heard about, were able to have my friend referred to then discharged to a full rehabilitation hospital. within 24 hours of acute treatment there my friend became a new person, and there they provide a true multidisciplinary approach.
these are just my concerns.
