Subject: Formal Complaint Regarding Incorrect Delivery – Reference ASW050641

To Whom It May Concern,

I would like to lodge a formal complaint regarding the delivery of my parcel, reference ASW050641, which was marked as delivered on Wednesday 24 June 2026 at 2:08 pm.

I received confirmation that my parcel had been delivered and, upon opening the delivery notification, noted that it had apparently been signed for. However, no one at our premises had received or signed for the item.

Our delivery address is Westminster Village Aged Care, a large facility with 101 residents and numerous staff members. After making enquiries throughout the site and confirming that no one had seen or accepted the parcel, I began searching the surrounding area.

I eventually located the parcel on a pathway outside Unit 2, 4 Sylvan Way, Grange, which is not our address. The parcel was left unattended in a public area and was at significant risk of being stolen. As the item is approximately three metres long and quite heavy, retrieving and carrying it back to our premises was extremely difficult and inconvenient.

I am also concerned that the delivery record indicates the parcel was signed for when, to the best of my knowledge, no one at our facility received or signed for it.

This situation is unacceptable and could easily have resulted in the loss of the item. I would appreciate an investigation into this incident and an explanation of how the parcel came to be delivered to the wrong address and marked as signed for. I would also like reassurance that appropriate action will be taken to prevent similar occurrences in the future.
