We sent the initial paperwork 2 months ago. Never heard a word. Called again, we don’t have it in our system. Sent it again, completed and sent back. Never heard from anyone. Called again and representative said “ it’s on the doctor’s desk, he will be back on Monday. Two weeks ago NOTHING.
Called today NO record of my husband’s paperwork.
Absolutely disgraceful.
