Affordable Dentures & Implants / Affordable Care review: Dental services
I paid for Dental services. In order to get reimbursed from my insurance. They have to submit a W9 form. This has been several months and the Affordable Dentures corporate office refuses to submit it. I have sent emails to the coporate office. The District Manager has not intervened. The Dentist office has been great and they have tried but they have no power.
Claimed loss: Thousands of dollars.
Desired outcome: The dentist office has no power to have the W9 form submitted to my insurance. Affordable Dentures Corporate office has to submit the W9 form to my insurance so I can get reimbursed for the thousands I paid out
