I am a physician on staff at NYU LI. As part of their vaccination program, I was offered to go to CVS. On 10/13/2023 I scheduled a high dose flu shot to be administered at the branch at 17 E Jericho Turnpike, Mineola, NY.
Upon returning home after the vaccination, I noticed the form provided to me to satisfy my NYU requirement was incorrect. It was for a patient born in 1983 having been vaccinated on 10/4/2023. The following morning, I returned to CVS in order to correct this mistake. Despite speaking to the pharmacist in charge that morning I was told that my proof of certification form could not be generated until the following day. I found this to be odd, but I was reassured that the corrected form would be sent to me overnight via FedEx. I never received this but did receive an email from CVS the following day, 10/15. A form was attached but with the incorrect date of vaccination, 10/14/2023.
At this point I have doubts that I was correctly vaccinated.
In the short term, a corrected form should be emailed and sent overnight.
Unfortunately, my confidence with this branch is tarnished and I only hope a root cause is investigated as physicians depend on pharmacies for competence filling prescriptions and advising patients.
