Quest Diagnostics - Quest diagnostics covid-19 test

Since 14 October 2020, I have been trying to get a quest to resolve this issue in vain. I made an appointment to get a COVID-19 test in Miami Gardens, FL. I paid Quest the $129.00 Invoice #692388. The lab rejected the appointment, stating that the Quest Reference # is wrong and they no longer honor Quest Diagnostics appointment. Since then, I have been giving the turn around, transferred more than 20 times to the same numbers, and Quest still does not refund the money I paid. Better Business Bureau could not reach Quest either. I have been told to send a letter to the state attorney on the issue.
I just need a quest to refund me the money I paid for a service I did not received

Desired outcome: just refund the fees I paid

Quest Diagnostics - Billing

Quest sent a bill for service on 10/5/20. The bill, sent on 10/24/20, was a PDF file embedded in an email that required a password to open. I accidentally entered the password incorrectly at which point the file locked up, meaning I could no longer access the bill information. As all Quest "help" requires a bill number, I am at a dead end. While my "MyQuest" account claims to include bills, there is nothing in it about my last two appointments.

At this point I need a bill I can read, preferably a paper one.
