I went to Massage Envy in Greenwood, Indiana last month. I filled out a cancellation form. I am a college student with very little money. Another transaction $65.00 came out of my checking account today. This has caused my account to be negative. I respectfully ask you to cancel my membership immediately and request a refund of $65.00 to my bank account.
