I ordered tests on-line, provided my insurance information as requested. They billed my insurance for 3 dates for "diagnostic labs" as though my husband actually went in to an office and had a covid test done by a Sajad Zalzala, MD in Gaithersburg, Maryland (we live in West Virginia). All I had done is went on a website advertised (and which I had actually used more than once to order at home Covid test during the whole covid pandemic). These were billed to my insurance, they were not paid by my insurance due to how they were billed, as an actual appt & the insurance was requesting additional information from the doctor to finishing processing the claim - wanting to know if he tested negative or positive for covid. I have been calling and fighting this since February 7, 2023, telling them it is for at home covid tests, nothing else & the federal government says that all covid tests are to be paid for and that I am not responsible for the payment for them. One of the UMR reps said that Mr. Sajad Zalzala has billed these fraudulently to get the money out of the insurance companies.

I am attaching copies where it whos it has been billed to my insurance, as well as a copy of where I have filed a complaint with the FTC.

By googling his name and/or the company name of Tiero, there is multiple law suits have been filed in different states from different insurance companies against this man & his company for insurance fraud.

Desired outcome: To not be responsible for payment of these tests. To make sure this does not go against me or my husbands credit or anything in a negative manner in any way.
