My father David Smith Jr was a patient brought into your hospital by ambulance due to a medical emergency of being hit on a electrical bike. He was taken into the hospital intensive care unit. He suffered a brain injury due to the accident.Unfortently he didn't have any medical health insurance at the time of the accident. After being cared for in Tampa general he was then sent to Rehabilitation center kindred. He was sent there from Tampa General Hospital from my understanding a grant through Tampa General Hospital for the time of 30 days. Within these 30 days there was an application for Medicaid submitted by David's daughter Kristine Smith. During Davids stay within these 30days of services at kindred rehabilitation service he has not received anywhere near acceptable level of care by this facility. He is supposed to received according from case management here at kindred rehabilitation center therapy 3x a week as of, yet our family hasn't seen that being done we do understand that his health would override that. At this point of my personal perspective of the care my father David Smith has received at kindred is all around just not okay he has been left in his own urine for long periods of time to where i witnessed myself him literally shivering from his whole hospital gown being soaked in his own urine an at the r time this happened during his visit i addressed the matter with his nurse at the time and she said that okay she would let her care team know because they would be the ones to handle things like that and I was understanding, Davids father an myself continued our visit patiently waiting as David still sat in his own gown full of urine roughly 45 minutes later David starts trembling due to him in a soaking wet hospital gown so I then walk out to the nurses station to get assistance because he was trembling so badly I thought he was having a seizure an there was no one there so I came back in an he was still trembling I took the soaking wet gown off of him an then wrapped him in a sheet to bring his body temperature back up. This isn't the first incident of medical neglect our family has noticed since his stay at kindred. Case management is reaching out to Tampa General Hospital to try to see if he will be approved for in home services due to him being sent to Pinellas County but is a resident of Hillsborough County in which is harder for our family to actually visit him as often as we would like to our family is understanding that David still is in a medical state to where he needs medical attention so we are asking for better medical services and we feel as if David would progress a little better if he was receiving home health care services. So, I kindly am asking Tampa General Hospital to please consider including in home medical care services for David Smith an outpatient service for rehabilitation services provided to him in home being he is under a grant through Tampa General Hospital as to him just being sent to another facility where he isn't being provided with the best of care, I am also asking Tampa General Hospital to provide any medical supplies and medications David will need along with transportation from kindred hospital facility to his home address at 11523 east bay road Gibsonton, Fl 33534 he will have two people at home that will tend to caring for him I am just asking for Tampa General to consider providing the medical services and supplies along with this to be possible.
Thanks,
Kristine Smith

Desired outcome: in home care instead of a facility with any medical needs for David to be available to him through Tampa General Hospital and any grants he may receive through Tampa General Hospital
