On 10/12, I had a medical visit at North Texas Orthopedics and *** in Grapevine. I asked for an itemized *** and a return to work letter at checkout. They said they would mail them to me in a couple of days. But, I still haven't received either one, even after calling and messaging them every day through the patient portal. They keep asking for my treating physician's date of birth to fulfill my requests, but I don't think that's necessary. I already have a signed medical release on file that's valid for a whole year. This isn't the first time I've had trouble communicating with this poorly managed medical office. The leadership is lacking, and the employees seem lazy. It's not right to use that as an excuse for not providing assistance. If they keep up with this poor leadership, I'll have to find another orthopedics medical office.

On 04/15 I went to North Texas Ortho & Spine Center in *** for elbow stem cell injections. I paid my full amount on the day of service of $500. On 05/13 I went for my follow up visit from my stem cell injection at the *** location. The day of the follow up visit I paid my copay of $60 for my services. Both times I was under the care of ***. I keep receiving bills for my follow up visit for the $60 of which I paid the day of. I've repeatedly contacted their billing department so that they can take care of this issue. I've repeatedly called, faxed, and emailed copies of my receipt from 05/13 for the $60 of which were paid at the *** location. I paid my copay with my flexible spending account card. I'm currently still receiving these bills for $60. I'm exhausted from calling repeatedly and no one has taken care of my issue. Every time I call, I speak with different people and they make it seem like they're gonna take care of my problem. I'm seeking for my billing charges to resolved so I can stop receiving bills in the mail of something which has been paid for almost a year ago. I have my receipts for everything.
