I report a potential systemic patient safety failure. A 74-year-old male presented to Grandview Trussville with stroke symptoms at 2:30 p.m. on May 5, 2026. After CT imaging, transfer to Grandview Medical Center, a certified primary stroke center, took about eight hours. An MRI ordered at 9:00 a.m. May 6 remained incomplete for over 36 hours despite the hospitalist’s efforts. It was performed about one hour after my third formal escalation. Staff reported limited MRI-technologist coverage, and I could not confirm on-call availability.

Desired outcome: I want an accrediting agency, health standards, public health standards or equaivalent to review Grandview Hospital quality assurance standards for a primary stroke unit.

My own opinion to people reading this is if you have any critical or acute life-threatening symptoms such as a stroke or heart attack, ask to go to the University of Birmingham Hospital or Emergency Room
