Concentra D.O.T. physical

To Whom It May Concern:

I went to the Bloomingdale, IL Concentra office on May 1, 2020 for my D.O.T. physical.
I had three blood pressure tests done: 150, 151, 158. I waited 5 minutes for the doctor; when she came in, she logged into the computer and 30 seconds later had determined that I had sleep apnea, even after showing her the sleep test (Merit) that I passed. It states "0" apneas, however, she was still certain this was my issue. She then asked me if I had taken something to pass (no, I hadn't of course). I stated that when I come into this room I get stressed and my blood pressure goes up. She stated that's not possible; that once again I MUST have sleep apnea. She told me that since drivers are under so much stress while driving that my blood pressure would be even higher when driving. I let her know that that is not true and that in my 37 years and 3.2 million miles of experience I set the cruise, turn up the radio and hit the highway. Our stress level isn't that high throughout the day. She let me know that I was wrong, because as she drives her car and people cut her off that she gets very stressed. I guess she knows what I go through because she drives a car for a half an hour a day. I showed her my phone that has at least the last 75 blood pressure readings (95% below 140) and she wanted nothing to do with that. She informed me that the week earlier she put a driver on a three month card for having high protein in his urine; not from a blood draw but a litmus test. She then asked me if I drink. I responded with I do once a week tops, 2or 3 beers (she thinks I'm lying) then asked do I drink and drive…What? I showed her all the test info off the DuPage medical app (My Chart); all blood test results, blood pressure readings and she didn't want to see it. I informed her that I had a 2d Doppler scan done (result normal) and she stopped me cold and wanted to know why my doctor would send me in for that test.
I responded that my doctors and I wanted to eliminate any possible causes as to why I had high blood pressure. She asked me if I had shortness of breath, and I said a little, but that I smoked for 35 years and quit 15 years ago. She said that I shouldn't have shortness of breath after quitting 15 years ago. She has no idea how much I smoked to determine if I would have shortness of breath (two packs a day for 35 years). When I told her that I also had a stress test (result normal), she once again stopped me cold to ask why. Same answer. I let her know that I went out and got a cardiologist to make sure I was doing everything I could to ensure I was covering all bases. He sent me in for a kidney vascular ultrasound test (renal artery US) to once again make sure that all veins were flowing correctly (result normal) and she couldn't really care because she wanted me to have sleep apnea in the worst way. She went back to "I must not be sleeping good and that is why my blood pressure is high". I again let her know that I sleep great on a high quality memory foam bed every night; and still, she believes that I'm lying. She asked if I get up to urinate at night; I don't, but once again she believes I'm lying because I'm over 60. She asked me if I snore; I said I don't, I'm a stomach sleeper, and she thinks I'm lying. Why ask the questions if you don't want the answers, or to never let me finish a thought. Very frustrating-blood pressure rising. I let her know that my D.O.T. card expires in five days and she responded that she needed more information and that I would have to go to all my doctors and get all their notes filled out and complete all her paperwork, and fax it in. I remind her that my card expires in five days, and two of those were Saturday and Sunday. I also advised that I had been home for 5 weeks on self-quarantine due to the pandemic and she didn't care. I needed to get to the doctors for their notes. She also wanted me to find a pulmonary doctor to re-read the Merit sleep test to make sure that I didn't have sleep apnea and, once again, I reminded her that I PASSED that test with 0 apneas and that the professional already read it. When she determined that I passed, guess what, she didn't care. I scrambled to get appointments to get her the information that she wants (not easy on short notice and a pandemic), but did. My first doctor was my PCP and he was quite annoyed and shocked that a doctor would second guess his decisions on my health in the 30 seconds that she had with me, and I've been under his care for 25 years. He did it, but did sit with me for 10 minutes to let me know that he was upset with her and that she seemed to be on a power struggle, as he and my cardiologist are actively working on this. I've been with my PCP for 25 years, have a physical every June and blood work twice a year to ensure my health.

My second visit, per her order, was my cardiologist. After explaining the details above to him, his first comment was "What is she doing? Trying to be a real doctor?" He looked at my home readings and adjusted my medication accordingly. He said that my blood pressure is good, and my kidney vascular scan looked great. He said many people have hypertension, for no other reason than just that, hypertension.
For your edification, I was forced to walk past a corona virus tent at Good Samaritan Hospital, to see this doctor. So much for home quarantine.
I then had to go find a pulmonologist, per her order. I then had a meeting with the pulmonologist. He did not understand why she would request him to re-read the Merit findings on my sleep test. He advised that the test was clear, and that I am not even a candidate for sleep apnea.
I think doctors should be able to determine if a driver is fit to drive. They should not be so hell bent on failing drivers to boost the facility's success rate of keeping fit drivers off the road to ensure they can stay in D.O.T.'s good graces, and have data to continue selling their services to other companies? We obviously don't know the answer to this question, however, this just does not add up. Why are my answers and doctor's tests/findings being ignored?

To have to jump through hoops like a circus animal and incur more cost ($25.00 copay PCP, $35.00 copay Cardio, $35.00 copay Pulmonary), not including my time and my insurance company's costs is extreme.

I have been a truck driver for 37 years. I am 60 years old, 272 pounds, work out 4 days a week (lifting, cardio), bicycle 7 months a year to and from work and most weekends. To have a doctor tell you that you are unfit to drive because your blood pressure is 10 points too high (not diabetes, heart attack, stroke). Ten points too high is wrong when you're under care with a PCP and Cardiologist actively working on the problem.

Any driver having to wait up to five hours to get a D.O.T. physical is wrong, and they know they're the only game in town, so why would they change it?

Every time they falsely give a driver a 3 month card, just to appease us, increased revenue for them every time, due to retest.
I am beyond frustrated and saddened at the process. I was nothing but honest with her, and she chose to ignore me and make me feel like a liar. Some doctors are accused of having the "God" complex; they can make us do anything why they hold our livelihood in their hands. This is completely inappropriate.
I spoke with Anna on Monday, and she advised to come back Wednesday to meet with this doctor. I went back today, Wednesday, May 6, 2020, 8:00 a.m., for my "final" appointment, and poor Anna advised that the doctor is out sick, and no one else can help me. Whatever doctor started this must finish it; Unreal. This is a clinic; why can't any on staff doctor read this and understand it? Just like paperwork I had to go obtain from MY doctors, why can't anyone read and understand?
At this point, I would like to make note that the only nice, professional and helpful people working there are Anna (front desk) and Rose, the manager.
Cheryl is rude and should absolutely not be working with the general population. She is completely unprofessional and brash.
As I had not heard back from Rose @ Concentra, I called and spoke to her and she advised that they did a six month card. Certainly not great, but better than nothing. At least she made an effort. She advised me that the doctor wants ANOTHER sleep study when I return in six months. I explained to her that not only did I pass my sleep test, but the pulmonary doctor that Concentra requested to re-read the test, determined NO FURTHER TESTING NEEDED. She advised that she said the doctor said you need another sleep study. How many doctors need to say no further action necessary before a doctor can understand? Is there a kickback situation going on? There has always been rumblings of this, but I never believed it—I do now. When I went to retrieve my six month card, I spoke with Rose and showed her the paperwork from the pulmonary that no further testing is needed. She agreed, and said she would put all information in my folder.
This has been a complete waste of all of our time, resources and money. I should never have had to take that sleep apnea test in the first place, but once again, they dangled my D.O.T. card above my head. Talk about stress. But not according to this doctor. I guess no stress to her; her job is intact. She was uncaring, rude, did not listen, and was downright mean. Flipped some forms at me and sent me on my way. This is no way to treat another human being.

Please act on this letter with the same time and honesty that it was written. Not all doctors are good doctors and this doctor obviously doesn't trust drivers, but we don't have anywhere else to go for our physicals and every year my stress level goes up. This is not due to driving, but because we have to walk through their doors and wait up to 5 hours to get this done and knowing the fight we have ahead of us over 10 points. It WILL raise your blood pressure (but again, I'm lying).
The thing that struck me more than anything through all of this is that I walked in there with good intentions. I was respectful at all times, ATTEMPTED to answer all questions, however, she in turn, treated me like a child. There was absolutely no need for this.
And please let us all keep in mind that this lengthy letter is not over diabetes, stroke, or heart attack, but all due to 10 points high on blood pressure.
I pride myself on how I work with my doctors to stay as healthy as possible. I ride my bike, walk and lift weights all for my well being.

Concentra random alcohol drug screen

Submitted urine.. Signed papers and went home.
Concentra called me 3 hours later at home telling me they forgot to do the alcohol test.
I could not return to concentra because I had to go back to work.
Concentra submitted a false report saying I refused the alcohol test and left the clinic.
I've been laid off work without pay trying to get this investigated/resolved.

Concentra wrong medicine

On 1/26/2020 I visited Concentra at 8755 S Harlem ave Bridgeview il for body pain+nose congestion
I told Doctor Koita that I suffer from prostatic enlargement and fatty liver though she prescribed me azythromycin 250mg and told me to get over the counter Advil cold and sinus but that Advil medicine affects seriously to people with health conditions I explained above

Concentra dot phy

I went to Concentra on a Saturday, Dec 2019. And the nightmare started first I thought it was only a name change, but found out after the exam the old company was bought out and my medical records was in storage and the clinic was unable to get them I also had a sore knee. the doctor (?) told me I would need to see a another doctor for my knee and heart condition and get a letter signed off for them (knee &heart).
Saw a doctor for my knee and got the letter signed and deliver it to the clinic and had my heart doctor office fax over a letter and test documents (the clinic lost the test doc.). Than I was told only the doctor (?) that seen me could issue the dot card. Went back on Saturday and found out that they said they did not get the paperwork and the letter the doctor faxed was no good (earlier that week I was told they had everything they needed) after raising my voice and pointing out this clinic was less than 2 star, and got a copy of the letter and test doc. which I hand deliver to the staff, after a couple of more days and raising my voice again some one signed the dot card for 3 months. At this point I was told get a new heart test and my dot card would be updated for 1 year.
Went and got the heart testing done, doctor wrote a new letter which I handed to them and was told it would be 24 to 48 hours to look it over, well over 48 hours later I stop at the clinic to see what was going on and was told they needed a copy of the heart test (no one on the staff call me or the heart doctor office to get this test). So even upset I got the test to them the same day. The next day I call to find out what was happening and was told I would have to pay for another exam.
Stupid me I did (sat in the exam room for over 1 hour) the doctor came in spend 2 minutes in the room found out I was the one that raised my voice at his staff and left (waited another hour) after coming back in he did the exam and that because I yelled at his staff I would need to see a shrink to be cleared (I realized 5 minutes into the exam he was not going to pass me) Due to the time spend dealing with this clinic I lost over a month of work (would you be upset no pay for a month?)
Yes I did raise my voice but I didn't swear or say I would punch some ones lights out. I did point out lost medical records, being MISINFORMED (could use another word), the poor job some staff members was doing and of cause the less than 2 star clinic.

Concentra service

At 6:08pm central time I had entered the facility with difficulty of breathing and informed them I was out of my inhaler sick and asthmatic. They took me to the back immediately to have a seat and said before providing me with a breathing treatment they needed to process my insurance and enter me into the system. Wheezing was consistent and hurting my chest because of the exhaustion.7:30 PM central time they had me in a paper wrap around to do a chest xray while still suffering the asthma attack I had come in with 1 hour and 30 mins ago. Almost 8pm (closing time) they came in with the nebulizer to set a breathing treatment after 2 hours of a server asthma attack . After the breathing treatment they said they were providing me with an injection /steroid however I was not informed of the medication I was injected with nor the names of medicine prescribed to me and told I had bronchitis from the doctor. They had us leave at 8:30pm central time after the injection with no discharge /or paper work of my visit with any information on how the shot would make me feel or what it was and how to handle my sickness. In panic of the burning in my arm I called another location that was open and a nice doctor in the franklin park location assisted with some questions though not all because my visit with the original location was not accessible. I am so upset on the service I had received, for an urgent care center it was not urgent at all and my lungs are incredibly sore for waiting 2 hours before relief. I called them 12pm central time today and they said they do not have any information on my injection and will have to call me back. No one should have to go through what I did last night.
The only nurse there that did her best to see me when she could was Debbi, the doctor was not around to often.

Concentra pre employment drug screening

i had received a job offer for USPS and when I went in for my drug screening at Concentra Medical Clinic in San Bernardino the clinician who took my drug test, "Kristy" messed everything up for me. I was ordered to take an Ecup Escreen123 Ereader 5 Panel Drug Screen. She called me to the back, and as she was explaining the steps for me she was searching in the cabinet for an Ecup. She pulled out one but it had already been open and had no lid. She then placed it on the counter and pulled out a brand new unopened one and also placed it on the counter. She had me wash my hands and then she gave me the Ecup that was already opened and had no lid and sent me to the bathroom. She told me not to flush the toilet and then left. She didn't give me any sort of time limit in the bathroom but I figured I can't take longer than a minute. So I quickly did what I had to do in less than one minute. I then carried my sample, without a lid, down the hall and gave it to Kristy. She grabbed it, placed it on top of the counter, still without a lid, and tried to get a temperature off it herself. By that time she couldn't, so she dumped it out into a regular plastic cup and waited another 1-2 minutes and still couldn't get a temperature. She quickly dumped my sample into the trash and told me " You probably took longer than 4 minutes in the bathroom that's why I can't get a temperature. So its going down as a refusal to test and as a failure. I will be sending your results to your employer". I asked if I could go ahead and take another test and it shouldn't be a "Refusal" because I didn't refuse. Also i didn't take no 4 minutes in the bathroom. She said " Right, but that would be for your employer to decide but for right now I'm marking it as a refusal to test and Its going down as a failure." She then asked me to leave. Now the Ecup Escreen123 Drug Screen is not suppose to be distributed that way. I was willing to stay there and give another sample but for some reason wasn't allowed too. In my results she put down I refused to give any sort of sample and that she did ask me to stay which that wasn't what had happened. If that was the case I would stayed without hesitation. Now I lost out on this job and can't apply again for 3 months ..thanks to this company and that employee "KRISTY" . I am beyond angry and confused on why and how that clinic can hire employees who ruin other people's chances of incredible jobs.

Concentra inaccurate specimen labeling

This story will seem like it's all about me. But bear with me; it's implications are far-reaching and it's creeping tendrils a possible extreme danger to society.

On September 11, 2019 I was in an accident at work, in a work vehicle. A tour bus hit my driver's side mirror. As per company policy, whether an employee is at fault or not, said employee must be drug tested. I was not concerned at all; I had no thoughts that this would be my last day at work. I was working in, and totally loyal to, a career in the automotive tech industry which I truly, truly loved.

I was driven to Concentra, on Polaris Avenue, by a shift lead. I provided a urine sample and was then seen by a doctor.

Fast forward a few days and I'm still not cleared to return to work. An email was sent to me from HR, essentially stating I could not return to work until they received my post accident drug test results. I thought this was strange because Concentra receives the results within approximately 20 minutes of the specimen being provided.

I started doing some digging. As it turned out, Concentra had sent my urine sample to EScreen in Kansas for 'confirmation testing'. At that point I knew I had tested positive for something. But how? And for what? I do not use drugs. Concentra tested me using a standard 5 panel drug test, which tests for: THC, PCP, opioids, cocaine, and methamphetamine, none of which I use.

A few more days went by, and I was finally able to speak with a doctor at EScreen in Kansas, who proceeded to tell me that I tested positive for THC. Not only that, but he said I tested at 40ng/ml, on par with a regular/daily smoker. I was stunned and angry. I told him that wasn't my specimen, or it had been mislabeled. He said 'We don't make mistakes.' I was subsequently terminated the next day.

I KNOW what I put in my body and I KNEW the test was inaccurate. It had to have been inaccurate at the start, at Concentra. I didn't understand. I went to my personal doctor and requested a urinalysis for THC. She sent me to LabCorp on September 20, 2019. I provided a specimen with a witness present. The results were negative, just as I knew they would be.

Now, if someone is a regular/daily smoker, as I was purported to be, THC is going to remain in the body for 30 to 90 days, depending upon the individual's metabolism and other biological factors. How did I test negative for THC just 8 days later? Well, because I was negative from the start. I propose Concentra either made a grave mistake, or they purposely and maliciously switched or mislabeled my specimen.

What are the broader implications for others, regarding my personal story? Consider the buffet of humanity that is sent to Concentra for drug testing: everyone from limo drivers and pilots, to policemen, firemen, paramedics, and all manner of public servants with whom we entrust our LIVES. Are they losing their jobs due to Concentra's negligence? Or even WORSE, are they keeping their jobs when they should be terminated? Can the collectors at Concentra be bribed or otherwise coerced into 'accidentally' switching samples in order to save someone's job? I don't know about you, but the thought both scares and angers me. The consequences are truly frightening and perhaps endangering our very lives.

Concentra poor service

I would like to speak to someone over the phone or in person about service I received on 10-14-2019. Service was extremely horrible and I didn't believe I got well enough care from Doctor James E. Belcher. I was brought in due to having a asthma attack and short of breathe.. I was taken straight to the back because it was serious. MA stated I needed a breathing treatment ASAP while my supervisor were giving them my insurance and ID information. The Doctor finally came in as the MA getting my information and ask what kind of medicine did he need to give me... I told him short of breathe I need a breathing treatment/ steroids or something.. I could barely talk. He never tried to check me out or anything to the wellness care checkup. I was given a 2 treatment on the request of the MA and oxygen. The Doctor only came in after the 1st treatment to say oh well you can talk now but I would get you another treatment because you are still wheezing. After the second treatment I was still wheezing/ short of breathe and then the doctor states I'm going to give you oxygen and release you. Then he asked for my inhaler to Write me a prescription. I said to him I still feel tighten in my chest and wheezing badly were you can hear if you where standing next to me. He assisted to say you had an expired inhaler get the prescription filled and have a nice day. Soon after leaving the urgent care I was taken across town to my own doctor and admitted in the ER. I feel the care I was given at Concentra should've been handled better so I didn't have to be rushed back to another doctor soon after leaving that urgent care..Due to that issue I'm forced along with my insurance to cover payment to additional doctors for care on the day for the same issue. Could someone contact me by phone at [protected] or send me information to go to a central office to speak with someone in person..

Concentra medical treatment

On 1/19/19 I went to the Concentra Urgent care in Los Angeles, by the airport, while I was on vacation. I woke up with an itchy rash with hives and swelling. Upon arrival we checked in at 7:00 am and continued to wait 30 minutes before seeing anyone appear in the area. It was then that a man appeared and said he was the Doctor but didn't have a key and he would find someone to help us. Shortly a receptionist appeared and we were called back to be seen at 8:05. Once in the room we waited again, the nurse appeared and then the doctor explained what was happening, he seemed very puzzled mentioned that I needed a few things and then left the room again. Never examining me. We waited for about 10 minutes and then my mom went to ask if someone was coming in to treat me because the hives and swelling were getting worse. The doctor came in to check on me and gave me a gown to put on because he wanted to check my body to see the rash. Again, he left the room and didn't return. My mom again, went into the nurse area and asked if I was going to be given any medication. The doctor was in the triage area on the computer and told her yes. The nurse and doctor came in and he said he said they were giving me two shots; one for the itching and another that was to calm the reaction. After administering the medication they both left the room, within 2 minutes I felt extremely dizzy, I then felt like I was going to vomit. My mom went into the hallway to tell the staff. I became extremely hot, and could not open my eyes, I felt like I was going to pass out or have a seizure, which I have never had. I was not in control of my body at all. I was then unresponsive. The doctor become frantic and did not know what he was doing, he attempted to take my temperature, which he could not get to work, and attempted to take my blood-pressure which he couldn't do and then called for the nurse to bring in a different blood pressure machine. The doctor was fumbling with everything and mother told him not to leave the room. I was completely white and my lips were blue, my skin was on fire and I couldn't move or communicate. A staff member called an ambulance. The ambulance appeared within a few minutes thankfully and they took over. Once in the ambulance I was able to communicate a little bit. They had to give me medication to counteract the medicine I was given. I was given too much. I was then taken to the hospital and treated, I was kept for a few hours to be monitored and given steroids, and fluids then released.

Concentra dot physical

I was given a 3 month DOT card she said because of my neck size no other medical conditions are present and was told that I need a sleep apnea test now, I cannot start my job and my bills are due. I feel like they are discriminating against drivers sleep apena is not required as part of a physical. People are being denied an opportunity to work because they are judging you by the size of your neck and BMI.

Was this in Round Rock, TX by chance? The concentra doctor told me the same exact thing. I've been working a highly physical job for over four years and have been keeping up with my DOT medical card fine. No health issues whatsoever. My cholesterol and blood pressure are perfect. She said that I need a sleep test, and that I was overweight (THEIR scale said I was at 216, but my scale and my doctor's office scale said I was 209 AND I am pretty muscular!). And that she would only issue a 3-month DOT card. WTF?

Concentra untrained medical assistants; poor technique with blood draws and specimen labeling

On August 7, 2019 this writer entered Concentra for a pre employment HEP B titer as well as a TB test. I am a Registered Nurse of 13 years, and I am always assessing.

One MA entered the room, and while her demeanor was professional and friendly, her technique of a blood draw was abhorrent. From the way she improperly applied the tourniquet, to the improperly fitting gloves, to the way she was incorrectly holding the needle and eventual improper stick. Everything you are taught NOT to do when performing venipuncture, she did to me. I have never seen such inexperience from any MA in my life. I had to help her find a vein, as she was clearly uncomfortable with the process. As I stated before, I am always assessing and I am very intuitive. I encouraged her to take her time and feel good about the location before sticking me. She then held the butterfly needle with both of her thumbs, and did not hold my skin taught when she incorrectly inserted the needle. I asked her to stop, and find another assistant to come and try again, as I was not comfortable with her technique.

Another kind MA entered the room and apologized, however I suggested that there was no need to apologize for something she did not do. I did express that I want someone who is properly trained in venipuncture. I have such a large and painful bruise from the improper technique of her drawing my blood. The picture below is evidence that she was also poorly trained. My tourniquet was left on too long, and she removed the needle before releasing the tourniquet. She then tossed the unlabeled blood in her supply basket. I asked her if she would please label the tube in front of me, and she said she would as soon as she got to the lab. I shared my feelings of discomfort, as human errors occur and that a specimen should always be labeled at the bedside. After she administered the TB test and incorrectly drew my blood, she then confirmed that I was the correct patient. Confirmation of the correct patient needs to occur prior to any invasive procedure. From my observations, she was rushing and uncomfortable with phlebotomy. To the lay individuals who experience situations similar to this, it is unfortunate the they may believe it to be the norm. I would never step foot in another Concentra.

Concentra dot physical

Ive bean driving 23 years went to concentra. In 6/2018 and got a 2 year certification i changed employers in may 2019 and went to get new physical and only gave me 3 months want me to do sleep apnia test because i have a 17 inch neck measured by nurse only. I have good blood pressure and vitals just also had stress test and passed and 33 bmi its just a money game its time they get investigated

Concentra is responsible for a lot of drivers loosing there jobs because of this practice... I was also done like this and has gotten a lawyer involved to file a class action lawsuit

Concentra tear in left wrist

June 3, 2019, I was sent to Dr Chan Concentra in Elizabeth Nj for a tear in my left wrist from Dr Patel in Concentra, Newark NJ. I saw Dr Chan today and was completely disrespected and refused medical from him. He asked what happened after the surgery because I have a brace on my left wrist. I said to him I have a tear in my wrist. He responded I have never saw you before. I responded and said yes you have. Sept 9 2014 because my left arm down to my fingers was numb from being hit with a chair from a student in December 10, 2013. He went to retrieve my full file. He asked why did I have surgery on my left arm I replied so I can have feeling back in my left arm, hand, and fingers. He asked who was the dr and I said Dr Zornitzer. He became very upset with me he also squeezed my elbow by the bone as hard as he could and said can you feel that I said yes when he released it my arm became numb going down to my fingers. Then he wouldnt look at the MRI that was tooken of my wrist didnt want to talk to me anymore and said Go BACK TO DR ZORNITZER AND HAVE HIM FIX IT! He left the room with the assistant looking red in the face she doing all the apologizing she can for his demeanor. I cant believe how he denied me treatment for second time cause I had to go to another Dr because he said there was nothing wrong with me. Concentra dot physical at mt. laurel, nj. concentra

Hello my name is John Martin. [protected]. 3 months ago I was giving a 3 month recertification card because the ME said my pressure was high even with him knowing I was on medication and has been that way for sometime. But he proceeds s to have me do stretching excercises than he checks my BP a course my BP will go up so I asked him to recheck it and he said no we only do it twice. On the day of my return for Re certification I stop in to see my primary and the the sleep physician. They check my BP it was fine. I even brought a recent two month old very good stress test in. My BP was good went I arrived at Concentra. They said they wanted my cardiologist to send them more paperwork. Then they wanted the results of my bloodwork. Then they dragged me out with this for a day and half before issuing me now with a 6 month card. I asked why it was stated to me by a clerk laticia. That the ME said I had too many heart issues. They are lairs. I had this for years and have a primary and a cardiologist doctor no problem. They are not to be trusted. They is something seriously going on with Concentra. They have changed and the truck driver is feeling it I do not like them anymore. And they are beginning to have this slimy filthy going on. I will be asking my HR to please stop doing business with Concentra and every driver I come across to go anywhere but to Concentra

NON-DOT Physical Deer Park, TX.
DO NOT list any pre-existing medical conditions for your physical. The doctor at this location is rude and will not take into consideration your doctor's notes and referrals. After several phone calls, requests for doctor's notes, and a note directly from my doctor stating my pre-existing medical condition would not cause any problems with the position I was being hired for she still failed my physical. The employer had also stated several times there was no issue with my medical condition and the position I was being hired for but the doctor still failed my physical. She mentioned several times job duties that were not listed in my job description that could be a hazard. I was not working with heavy machinery or driving a company vehicle but she still kept making it a point to reference this when I was asking why would my physical could be failed.

In 1/2018, a Dr. working for the Concentra Oregon Ohio Office "Claimed" I needed to be tested for a pre-existing medical condition before he would issue me a DOT Medical Card. That was he did 10 minutes before I asked him why the other THREE Doctors that examined me days earlier ALL signed statements saying they saw no reason I couldn't work. The Concentra Dr. then decided I didn't need the test after all. Then changed his story to I needed to come back in 6 months. The Concentra Dr. had no way of knowing I am a former Army MP who is very familiar with the kinds of cons, fraud, and scams used to victimize Veterans. Especially the one known as "Lie and Deny". Where you simply lie to someone to get something from them. Then if you're caught in the lie, you just deny you lied. And claim the person you lied to just didn't like what happened as a result of the lie you told him. Very hard to prove without a witness to the lie at the time. Next DOT Physical I will do what I did last year and take my Atty with me. Funny thing, I had no problem getting a DOT Medical Card last year. Just a co-incidence I'm sure.

Concentra workers compensation injury

I work at a Pre-K school and it involves heavy labor of pushing large tables, picking up chairs, pushing shelves in place, bending down and sitting criss cross applesauce, picking up kids who are swinging and hitting on you, picking up children to change their diapers, physical play exercise etc. This is done every day. So the day when I picked up a Student to change her diaper and picked her up to prepare for a parade, and Oh yes the items above I also did to my muscles in my chest became strained and grew worse over the weekend. Dr. Fernando made it appear as though I always had an injury in that area that wasn't true. The injury wasn't work related even though the medical signs mentions a person who constantly uses the same over worked muscle or one who over exercises. To make me appear non-compliant she had me scheduled to come in at 11:30. I came in at 11:28 and she just walked passed me constantly and then saw me at 12:28. Prior to that time when she knew I was there she told the physical therapist Jangle and they both said I had missed a second appointment for my physical therapy counting that time bc they both knew I was suppose to see the physical therapist at 12pm. They knew there was no way I could see the Physical Therapist if the doctor was seeing me later and at that time. Then she wrote I was non-compliant and had missed two days. These ppl are very shady and plot scenarios to make things appear a certain way on paper. If u have a legitimate injury don't come to this clinic bc I think they're working for the other side and they could really careless about ur injury or the truth.

Concentra concentra dot physical

All my vitals were good I had two years before. Doctor give me sleep study for 3 months dot card. He wants me to do sleep study. Just because of my neck was 17 but my bmi was high but im big bone blood preasure is good 137/78. He was short with me and there was a dirty needle on the floor ive could step on I had to tell him to pick it up. Not happy I need a 2nd dot doc and help with this matter. I never had sleeping promblems no history

Concentra untrained medical personnel

My daughter, who is in recovery ( drug addiction) recently had a back injury.
She had to see a doctor at Concentra, as her injury was work related. She
was given a prescription for a muscle relaxer. She did not pick up the medication, & explained to the doctor why she could not take it. He dismissed her concern, & told
her she should have taken it anyway, since it was a very low dosage. We were all shocked at this physicians complete ignorance & dismissive attitude. How could he
be so ignorant ? Thank God my daughter knew not to take this medication, as it
could obviously have led to a relapse.

Concentra doctor

I went to Concentra clinic with a worker's compensation injury. The doctor there, Warren G. Hatley, was very dismissive and not concerned at all about my pain. I do have plantar fasciitis and as soon as he heard that he quit listening to me and told me this injury could have happened anywhere and was related to my plantar fasciitis. The pain I was experiencing was on the top of my foot going up my leg. Nothing like plantar fasciitis. I went through my own insurance to a podiatrist who diagnosed me with a strain of muscle and tendon of long extensor muscle of toe at ankle and foot level. I was put in a walking boot for 4-6 weeks. I feel as if the doctor I saw was negligent. He should have seen this injury or been listening to my complaints to refer me to specialist.

Concentra dot physical

Dr Willam Curry fresh out of medical school decides and makes up rules and regulations that are not law. You pass all requirements for DOT physicals for 35 years and now I get a doctor that does not like that, I take a pain pill and sleep medication at night so I can sleep and have been on other doctors care for the past 15 years because of a kidney disease. Do not take the dugs during work hours and it would be like drinking and driving if I did but It has been the same for the last 15 years I have done this and when I asked when the law has change and I have found out they have not and he has out rite discriminated against me and against every one else, last time I checked the federal government sets the rule and regulations for the DOT physicals not this clinic and or doctor. Law suit is being concedered so this will never happen again. This happened on 07-06-2018

Concentra neglect care from dr. signe h. o'neale md during visit on 06/18/2018

I was seen by Dr O'Neale on 06/18/18. I fell at work on 06/13/18 and was sent to Concentra by my job HR Department (Care N Care, NTSP). Dr. O'Neale and staff was in a hurry to get patients out because it was close to the end of the day. The staff was unfriendly and rude to me and other patients. Dr. O'neale neglected to exam me correctly. When she entered the room she did not introduce herself she just went to sit down on the chair and started to ask question like why are you here. I asked are you the doctor or the nurse because I wanted to know whom I was speaking with. That's when she introduced herself. I told her that I fell at work in the rest room and my back, knee and head/shoulders was hurting. She asked when did I fall, I told her 06/13/18. She asked why I waited so long to come there. I told her that when I fell my supervisor took me to Texas Health Harris downtown Fort Worth and I was seen there and my HR Department person told me to come here to be looked at. She then asked me where I was hurting. I told her and she was writing on her pad I guess everything I was saying. Then with out her examine me, she got up and walked toward the door. I asked her if I could have something for pain and she said that the nurse that gives shots has left for the day and there was no one there to give me a steroid injection. Then I asked if she could give me a prescription for something for pain, then she said didn't the emergency room doctor give me anything. I was truly shocked that she said that and treated me that way. I then asked her if she was going to examine me or order x-rays or some type of MRI or Scan or something because I was hurting and my head was hurting and has been since the fall. I had to ask her that three times and she ignored the question at first and she then told me that the x-ray person was gone for the day and for me to return to the emergency room and have the doctor order it for me. I said to her, why do I have to go there aren't you the worker's comp doctor, I don't understand why I have to go there and why you not going to examine me. She just walked out! A few seconds the nurse came in and told me to put ice on my back and take some Tylenol for pain. I was upset when she told me that. I told her that I was going to report them and asked her for the complaint # to file against the doctor. She told me to hold on. Then the doctor came back in and asked me to lay down on the table and she lifted my left knee and put it down and the had me sit back up, which I was having a hard time sitting up because of my back hurting and she pushed on my back really hard while doing a half "[censored]" examination and I told her that that was hurting a lot then she stopped and said "ok" and the walked out and the nurse came back in the room with two syringes full of some liquid medication and said that she was going to give me a steroid shot and some Torcol injection and she told me that she was going to do the x-ray on my back. I said to her "I thought the nurse that gives medication injections was gone for the day". I also reminded her that both her and the doctor told me that the person was gone for the day. I went ahead and let her do the x-ray and give the injection because I wanted to know what was going on with me and to get some pain relief! I'm not sure if she did the x-ray correctly and felt uncomfortable. I went back in and in about 15 seconds the doctor came in the room immediately and told me that there was nothing wrong with me back that she could see and for me to wait out side and she will have my paperwork sent up there. I ask if she was going to order something else for my back and head and examine me. She just said maybe therapy and walked out. I went back up front upset and feeling medically neglected! After waiting a while up front to check out, I went to desk and asked reception/office clerk when will I get my paperwork. She turned around and picked up the papers from the table behind her and gave me the papers and told me that I can return back to work full duty. Now, at this point I'm confused. You see while I was trying to explain what had happen when I slipped, I also informed Dr. O'Neale that I was FMLA already per my primary care doctor Dr. Huy Nguyen and that I went to see him because my human resources person had not called or emailed me back on where to go for the worker's comp accident yet and he took me off work for a month until my next visit with him on 07/13/18 and that he ordered x-ray of lumbar spine and CT Scan of the head and informed that he wanted me to do therapy once he gets report back. I even showed the order to her when I asked if they could do it there and she told me that the person that does those things was gone for the day. So she knew that and she still put in her report that I could return back to work working 8 hours or more and can lift no more than 30 lbs. for up to 3 hours a day. She totally ignored everything I informed her of and was just rude and ignored my pain and neglected to give me proper examination, care and proper medical treatment. I also need to mention that when I was seen by Dr. O'Neale at Concentra I was on crutches and leg brace and in pain at time of visit with Dr. O'Neale. I informed her that I received the equipment when I went to the doctor from the emergency room at Texas Health Hospital put me on crutches and a knee brace to wear. Also my PCP Dr. Huy Nugyen also told me to continue keep using the equipment as well. I also emailed and spoke in-person with my HR Department (Debbie Schreier) and informed her of the incident and asked her to report it to my worker's comp agent because I did not have his information yet or spoke with him yet.
