USA Clinic Group / USA Vein Clinic review: Abusing my credit information and illegally charging my credit card

In response to an TV advertisement of USA Vein Clinic I contacted them at [protected] address 156 Rt. 59 Ste B1 Suffern NY 10901. for an appointment due. This was my first time using this clinic. My appointment was 6/7 at 1PM. When I arrived, I completed the registration information and provided my insurance card. I was told by the receptionist that I would have to pay my co-pay of $40. Which I gave them my debit card information. I was never told any more about the amount this visit would be and any additional charges. I never received an invoice or bill but Needless to say, I found out my insurance did not cover the full amount. I only found that out because USA Vein Clinic/USA Clinic Group ran my debit card for this amount without my approval or knowledge. I reported this to my bank and told them I DID NOT give permission for them to charge me this amount. It was not until contacted them 9/22 to complain about them charging my card without my approval my permission I received an email with the an invoice stating my insurance did not cover the full amount. Prior to that I was not informed or notified.
