Crunch Fitness

I joined January 2023, Crunch Fitness 5218 S. Florida Avenue, Lakeland, Florida 33813. Within a month or two my right hip had to undergo medical treatment. During this time, I requested a pause. The pause lasted till May 2023. I notified Crunch Fitness in Lakeland, Florida and they requested a doctors note to extend the pause. I supplied the letter stating my hip issue. On June 17, 2023, I was charged $28.99 for my membership that was supposed to be paused. The manager and employees I spoke with were rude and did not send the refund form. I want my $28.99 refunded and to cancel my membership immediately.
