Smart Start is required to provide the NJ MVC a Certification of Installation for my installation on 4/30/2025. I have requested the form be sent multiple times with no response. Can't get a definitive response on the phone. Without the form, I cannot have my driving privileges restored and therefore cannot use the vehicle. I submitted a request on May 1 and still no form has been provided. Additionally, Smart Start has refused to provide me the form when NJMVC states that they would expect the form to be provided to me as it is by other customers of other interlock devices.
Desired outcome: To have the Certification of Installation for NJ MVC sent to myself and NJ.
