poor baby...get a job instead of relying on CS to fund your life. you worry about you. let the CS agencies worry about the legal stuff
working in the insurance biz, i can honestly say that your problem is common when people do not understand how insurance really works.
most insurance providers do not even go by PPO or HMO anymore. the type of plan that you have has in network and out of network providers and you are able to use a provider of your choice, but your benefit level will be based on whether or not that provider is in network or out off network. for the best rates, you will always want to stay in network because they are contracted with that insurance company. Providers that are out of network can balance bill you anything the insurance company does not cover.
you should always check your benefits, network status, and any other prior authorization requirements before having services rendered. All insurance companies make claims, benefits, and provider searches available online these days with the grace of the internet. not even to mention the ability to verify all of this as well with their call centers. if the claims have denied due to insurance error, the representatives have absolutely not problems fixing them if they are legit. Representatives are a working employee just as you are and gain absolutely nothing by not assisting you. if the claim is correct, it is correct based on the guidelines of the policy that your employer offered you and the policy that YOU chose. any and all insurances are not charities. they process things off of the guidelines set with the policy that you have chosen.
the bill that you say you received is definitely not from United Healthcare. United Healthcare and BCBS do not bill for services you receive. all billing by these two insurance companies, and i believe Humana as well, come directly from the provider that rendered the services.

"Some people just can't be pleased." you sure hit the nail on the head...this would also include you since your complaining about not being able to find one to coddle you.

home depot, although they have what you want (the coddling employee to give you attention), are too much. if the employees arent busy, they are stationed at the end of an aisle ready to ask you if you need help. its kind of annoying when you know what you want but every aisle you pass, you get asked if there is anything you can be helped with. NO FOR CRYIN OUT LOUD, FOR THE 5th TIME, I DONT NEED HELP. competent people can ask for help when needed. and seeing as they have dozens of employees working during any point of a day, if you cannot find an employee, you arent trying hard enough

this is laughable..."this man makes well over $100, 000, and is the military". doubtful unless he is higher in the food chain.

"it wouldnt go onto his credit, and because of that he knows they will not wage garnish" - like hell they wont. if you are behind on payments for up to a year, they will garnish wages.

"and now their gonna have a grand time finding him!"- im sure they wont have a hard time finding him. they never do. although he may be hiding, i would assume it also means he doesnt see his child which is a poopy situation in itself. however, he is entitled to his privacy and security. if you dont know where he lives, thats between you and him. but, im sure they know where he is. they dont have a problem tracking people down if they are on the grid at all- IE jobs, credit cards, car payments, etc.
thats why they keep the cleaning bottles around. should wipe them down first

actually, Ingenix is the company that United Healthcare works with for things such as fraud and subrogation.
In some states, it is mandated that auto insurance covers as primary. when a health plan covers as primary, but the auto should have, ingenix will work in conjunction to UHC to regain the funds that they spent during the time that auto coverage was not. You specifically do not have to pay the funds back, but the auto carrier should be working with Ingenix to return the funds for the services that they should have been covering. .
your plan says 100% for preventative exams. preventative services are defined by the what governments preventative task force determines to be considered preventative services. During your annual checkup, if you had a blood test for things such as STD's or other such things, they are not going to be covered under the preventative benefits even though it was done during your physical.

This is really the case with the blood tests that were done. insurance companies have to process what they recieve based on your benefits. if they are not covering it under the preventative benefits, its because it is not an eligible preventative test.
if you did not ask for the tests to be done, why would you blame the insurance? the insurance processes what gets sent to them. if anything, you should be mad at the physician for ordering the tests. insurance companies dont get paid for the tests that get done. the physician does
being a PPO for the insurance still doesnt make your insurance at fault. contrary to popular belief, physicians and insurances are not in together on this so called global scam to rip everyone off as you seem to believe. if the tests werent requested by you, i would highly suggest that you dispute it with the physician or the group tha he/she is part of

you knew it was "cheap". thats why you bought it from that site to begin with. if the price is too good to be true, it is. the cost of the jerseys are over a 50% discount. did you expect real?

also, a key thing to look at is spelling, punctuation, and grammar. if there are massive or multiple errors, it is another sign that it is a chinese knockoff. there is a huge mistake right on the main page. "Reisger" aka: supposed to say register

if the price is too good to be true, it is. you knew they were cheap because of the price you would have bought it for. it has over a 50% discount.
they asked you why the waitress was watching you eat because it seems fishy. why would anyone sit there and watch a person eat instead of doing the work they should have been doing. they find it a little odd just as we do.
i have never gone into a restaurant and had a waitress watch me eat. believe it or not, there is a lot of fraud that happens. they are going to investigate it to rule out fraud

D-Castle, do you happen to have the number to that pasco office? (i now live out of the state)
ive been paying my child support for many years and trying to do things the right way. they require me to supply insurance, which i have, but to notify them of it is damn near impossible. i would run out of minutes on my phone before actually getting in contact with the child support enforcement number for the state of florida. the prompt when calling in is "you wait may be up to 30 minutes".
they really need to revamp their system to be more user friendly

the child support enforcement seem to be all about just helping themselves and the custodiial parent. they will do everything possible to get your money but dont really make it easy to do the right thing if you are the payer.
their website offers the e-service. however "Parents ordered to provide support cannot sign up for Child Support e-Services at this time". wtf. why wouldnt we be able to sign up for it.
i suppose all of these techs on here are made by "Barrister critisizer and competitors."? according to jessica, they are.

when you pay up front, it isnt because of the insurance. providers have the legal right to request payment up front to secure at least part of the cost. your lucky you only got charged 300.00. CT scans cost alot more than that. depending on the provider, they try to secure at least a percentage of the cost incase your insurance doesnt cover the service for whatever reason. Most times, they base it off of where you are with your deductible or out of pocket maximum.
and btw...blood tests are covered regardless of where they are done. your physician is giving you a line of BS
exactly, you pay a monthly cable tv bill regardless of whether you use it. a premium for insurance is to pay to have the luxury of being insured. it is a monthly charge. plain and simple

it really is true that cheating and ways to cut corners, gets you exactly what you have coming.
the reason for the pre-existing condition investigation is to find out if there is a pre-existing condition. these types of investigations are done to ensure that people are not getting insurance to cover something that they were recently diagnosed with. with that said, i am assuming that you just got the insurance within 6 months or so of your first visit.
The investigations are done based on the diagnosis that the physician lists. if it is something that deserves the investigation, the insurance company will do it.
before you think of how greedy the company may be, think about how much money they spend in a day. also, if this is a policy that you got through your employer, you're actually paying your employer and not the insurance for the account you have.
copay amounts do not benefit the insurance companies, nor does a medical insurance determine the tier level of a medication.

i can whole heartedly tell you that United Healthcare does not need referrals. Physicians that you use may need them, but the insurance does not
